Oral antifungals are ercommended

Detailed Answer:
Hello Dear

Welcome!
I am Dr V XXXXXXX a Dermatologist.

I have noted the details and would be glad to hep you.

From the details, the is possibility of a fungal infection is strong.

In such cases, I advise to adopt following measures and invariably and the results are very encouraging

First, one needs to stop using other medications for local use that contain steroids in combination with antifungals. If single ingredient Miconazole/ Clotrimazole is being used, then it is fine but these seem to be not leading to the desired improvement, as described.

Keep the part dry, ventilated, and wash the area thrice daily using plain water followed by thorough drying

Terbinafine cream locally twice daily along with tablets of the same salt, 250 mg twice daily for two weeks

Zyrtec 10 mg or another antiallergic daily.

In three to four days, one may start noticing the positive changes. One should also give rest to the part by avoiding heavy exercise or physical activities for a brief period.

You are requested to make the best use of suggestions and also contact me anytime for further clarifications

Best wishes
