Suggest Treatment For Restless Leg Syndrome And Symptoms Of Obstructive Sleep Apnoea

Question: I just received an answer to a previous question I posted about daytime hypoxia along with diagnosed nocturnal hypoxia. All the recommended tests have already been done with negative results. But the symptoms are of course still there & experiencing extreme sleepiness also. Sleep deeply at all times. Sleep 8-10 hrs at night & have to take approximately 2 1/2 hr nap daily also. Body shuts down. Have fallen asleep while eating. Have had MRI for neurological also. Tests say everything negative except nocturnal hypoxia but we know the daytime hypoxia exists with definite decline in cognitive abilities. We also know a test exists to determine this but can't find out the name of it for proof. How can we find this test name?

1. 57
2. 5'2" & 190
3. Was heavy & quit 2007
4. Was long haul truck driver with husband for 15 yrs
5. Have hypothyroidism
6. Snore a lot
7. Have had full sleep study. Tried to do without oxygen but alarms kept going off so had to put back on. Only diagnosed with restless leg syndrome. Medicine makes me even sleepier so quit taking. Had full heart workup with stress test & overnight thing. Had chest xray then sent to Neurologist. Did MRI. Kept asking if had accidents or fell on head but just now remembering being knocked out cold several times but think that is all. Was sent to Chapel Hill to specialist. Did another set of lung function even though we told them wasn't lungs. (Have mild COPD w/emphysema). Then sent to Neurology. Said have Mild Cognitive Impairment.
My symptoms are like being at high altitude with no oxygen & then get oxygen & back to normal for most part. Have extremely bad memory. Has been joke for numerous years. But there is no rhyme or reason as to what I remember long term or short-term.

Your history, your symptoms and signs are favouring Obstructive sleep apnoea ( OSA ) only.
But since your sleep study is normal, I want to ask few more questions for alternate diagnosis.
1. Which medicines you are taking for thyroid?
2. What is your last thyroid report? (With date).
3. Have you ever tested ABG (Arterial Blood Gas ) analysis, both during night and day?
4. You write about oxygen being given during sleep study. Was it through simple oxygen mask or CPAP mask?

Follow up: Dr. Drkaushal85 (15 hours later)
1. Levothyroxin - 50 mcg - 1 tab 1x day in am
2. 4/16/2014 - normal
3. No ABG at all
4. Had already been diagnosed with nocturnal hypoxia & on oxygen at night. I wanted to see what results of sleep study would be on normal basis but ended up having to add oxygen back. Was just through canula as normal.
Also diagnosed with Major Depressive Disorder, Anxiety & Bipolar. I requested nurse not to record diagnosis because intended on going back to work but wasn't able & new nurse disagrees with that diagnosis & won't include as current.

Hello dear, thanks for your follow up question.
For the 100% correct diagnosis of hypoxia, ABG (Arterial Blood Gas ) analysis is must. The value of PaO2 should be low to stamp it as hypoxia. Simple pulse oximeter will not 100% true. So ABG is must in your case.
And sorry to say but your question about sleep study was not clearly understood.
What you exactly want to know?
Expected Sleep study abnormality in nocturnal hypoxia?

I didn't have a question about sleep study. This was my sleep study. Tried to do w/o oxygen but had to go back on. Thank you for all your help!

you have strong possibility of OSA.

Hello dear, thanks for your follow up question.
In my opinion you are having strong possibility of OSA (Obstructive sleep apnoea ). As you are having all the risk factors.
If possible, I will suggest to repeat sleep study with titration with CPAP(Continuous Positive Airway Pressure). And if your nocturnal hypoxia improves than it confirms OSA.
