Suggest Treatment For Retrobulbar Neuritis

Question: I got diagnosed with retrobulbular neuritis 4 days ago since I lost vision in my left eye. The MRI is normal. The ophthalmologist suspects Multiple Sclerosis. I have been on Encorate Chrono 500 mg twice a day for more than 15 years now. How I can I regain my lost vision?

Hello,
I have gone through your question and understand your concern.

Retrobulbar neuritis at this age needs to be evaluated for multiple sclerosis and neuromyelitis optica.

If MRI of brain is normal, MRI spine should be done too.
CSF examination for oligoclonal bands and serum for nmo antibodies needs to be done.
Visual evoked potential should be tested.

Other investigations can be planned based on these tests' results.

Methylprednisolone for 3-5 days 1 gm intravenous should be given for vision recovery.
Early treatment provides good prognosis and recovery.

I am enclosing the MRI images. It seems that it is preliminary MS. How far the treatment you suggested is effective and what are the side effects of Methylprednisolone? A delay of 2-3 days in taking the treatment will be alright?
Expect your expert opinion. Although there are no preventive measures, since you are specialist I wish to know any precaution to be taken.

Hello,
Mri of brain is not normal and multiple lesions are present.
You need to visit and consult a neurologist rather than ophthalmologist for further investigation and treatment on line of multiple sclerosis or other demyelinating disorders.
Methylprednisolone should be started as soon as possible because recovery of vision is dependent on early therapy.
Methylprednisolone can cause gastritis or weight gain.
For prevention of recurrent attacks, avoid recurrent infection and use of hot water bath.
Take ample sunlight exposure and vitamin D supplements.
Take treatment under a neurologist.
