With this in mind I can think of the following causes for a pain in right lower abdomen with low back discomfort, side of abdomen and around waistline:
Colitis of the cecum also called a typhlitis.
Retro-cecal Appendicitis ( retro meaning behind the cecum).
Ilio-psoas abscess. ( muscles at the back ).
Urinary tract infection is rare situations.
I would advise you the following:
Blood- CBC, sugar, urea, creatinine.
Urine - routine, microscopy, culture and sensitivity.
Stool examination.
Repeat ultrasonography .
CT scan to confirm or rule out specific cause.
A course of an antibiotic, metronidazole, probiotic and so on.
If there is a complete response, nothing more needs to be done.
If surgical cause is found, it is better to get operated.
