This may be a problem called balanoposthitis.

The effective treatment is as follows:
Clean the area with plain water 2 to 4 times a day and and keep it dry with a pure cotton cloth or a dryer.
you may apply liquid Povidone Iodine locally or a Metronidazole cream- nothing else at all. We have seen no results with any other solution, cream or ointment. So do not try anything else; if you are using any, please stop it.
A course of antibiotic with metrogyl orally for 5 to 7 days should help you to get a cure. It is advisable that your partner also gets same course.

With the history you have provided, I would go for the first diagnosis of balanitis ( only on glans and posthitis is also on the foreskin). Please take the treatment accordingly.
The picture without the proper history looks similar to syphilis, but in Syphilis you do not get a skin tag, ulcer is punched out and painless and firm to touch.
On a safer side, take a consultation of a Dermatologist and Venerologist to get this confirmed and to get a prescription of antibiotics.

Syphilis test from pharmacy are not very specific and sensitive.
Syphilis can be contracted only on unprotected sex with the person who is infected and your history is not suggestive of this. ( Your history : It started off almost looking like a tiny bit of additional skin (almost like a skin tag) with a tiny bit of light pinkish redness around the skin tag. unfortunately picked at it AND protected sex with a condom on XXXXXXX 13th with the mother of my child).
