Hello welcome to HCM.
I have read your question and understand your concerns.
The 2d echo report you have mentioned has mild age related changes only except mild AR(Aortic regurgitation).
Aortic regurgitation is also mild severity, so it has no great significance at present. No any extra medication is required for that.

My advice to you is -

1. Grade 1 diastolic dysfunction is usually found in persons more than 60 years of age. You might feel breathlessness on heavy exertion because of it. Regular daily walking and regular medicines for blood pressure and diabetes is the treatment for it.

2.Mild AR is also seen sometimes in old age, as such no extra precautions to be taken. Only repeat your echo after 1 year to see if any further changes is there or not. Aortic valve sclerosis andtrivial TR is also nonsignificant.

So, in summary your echo report is near normal which is actually nice.

3.Continue the medicines you have mentioned for diabetes, hypertension and high cholesterol.
Kindly do your lipid profile, thyroid profile, kidney profile, blood sugar levels every 6 monthly.

Hello welcome again..
I understand your concerns about diabetes.

Do your FBS, PP2BS AND HBA1c report every 3 monthly for adequate sugar control.

HBA1c is a 3 month average sugar report and highly reliable as in overall diabetes management.

Usually we allow following values in our patients of diabetes.
FBS <140
PP2BS <180
HBA1C <7

If your 3 monthly levels are below this levels mentioned then you don't need to worry much about your future diabetes related complications. So, no need to become anxious about it.

Blood sugar levels will fluctuate all the time. That is normal. How much fluctuations are there that is more important actually.

In future if your blood sugar levels remains highly fluctuated then I advice to take extra long acting insulin at night after a prescription from your doctor. It will maintain 24 hrs blood sugar levels. Other dosages can be adjusted accordingly.

Regular daily brisk walking for 1 hrs and strict diet control are very important in diabetes and hypertension management.
