I have had the issue of itchiness and tough skin (rarely is scrotum soft). The sack never seems to hang. Lately I tried not to itch. Scabs formed and then a lot of discharge

Hi
You seem to have developed scrotal neurodermatitis which is an intensely itchy eczema of the scrotum .
Now the most important thing is to stop itching or scratching .This eczema tends to increase with scratching,so the more you scratch ,the more will be the itching .
To prevent itching ,keep the area moisturised by applying some good moisturisers two to three times a day over the affected area .
If the urge to itch is too much ,keep a cotton cloth dipped in cold water over the area so as to suppress the urge .
In addition ,you might need to be treated by application of steroid anti fungal combination creams over the scrotal region twice daily .
Regular courses of antihistamines will also be required .
It is an easy to treat eczema but it tends to recur if the person continues to ten or scratch ,so you really need to divert your attention to something else when you have the urge to rub.
