Suggest Treatment For Seborrheic Dermatitis Along With Dandruff Problem

Question: I am repeating the reply again. Sorry in case you have have already got the message.

My hairfall started 12 yrs back in 2003 during winter season when I suddenly noticed extreme itching and hairfall on my left forehead. Strangely other areas of scalp were unaffected i.e. no dandruff no hairfall.
Slowly over a period of 4-5 years it spread over my entire scalp. Also my skin became very oily and mild pimples on my face became a norm.

So first time I went to a dermatologist was back in 2008 when he said that i suffer from MPB and s. dematitis both. He gave me propasalyc NF(to be used everyday for 2 weeks in night), candid-B(2 weeks to be applied in night before sleep), minox 5% and fin 1mg. He also gave some folic acid capsules and some fluconazole tabs.
Strangely I noticed change from the very first application, the dandruff was absent and so the hairfall. No matter how much I combed, pulled or washed my hairs the dandruff and hairfall were absent.
But the effect reverted after 2-3 weeks.
And I never noticed improvement after that even though I continued with medication for 3 months apprx and finaaly decided to quit as my doctor was only changing the sampoos and some mineral tabs.

Then I again consulated a doctor in 2011 this time he told that hairfall cannot stopped it can only be prevented or delayed. He gave candid-B with some tar gel & one tar+salisalyc acid shampoo.
I used that also for 3-4 months but noticed very little change.
So that too I quit.

Now in 2013 with the onset of summer, I got acne burst on my face. But the isotretinoin that I ate made my dandruff+hairfall worse.
So I decided to consult the same doc whom I met in 2008 for medication and he prescribed the same course minus propasalyc NF. It showed improvement for 1-2 weeks then effect was again gone. I continued with his meds with 1 yr approx but as a sideeffect of fin tabs I started having less ejaculation and loss in sexual desire. So after one year I decided to quit this also.
Also since starting the course I have been washing my hairs with Sebamed shampoo sometime head & shoulders too.
I still have acne MPB and dandruff + too oily scalp and skin.

Please suggest me the course of your remedy and dietary changes too if needed.
I also want to know if my condition is genetic or I have got some infection like psoriasis or so.

One more question I want to kknow is about ozone treatment.
What is your opinion about that. Shall I give it a try?

Welcome back and thanks for your detailed response.

I can understand your concern for the problems related to hair and seborrheic dermatitis. As a young man you are quite worried about all the things related to your scalp.

After reading your query I can make out two basic problems- one is hair-fall and the other is dandruff or also known as Pityriasis capitis. When this dandruff is very high then it leads to the problem of seborrheic dermatitis. Both of these problems are quite common in your age group persons.

Now taking all the problems one by one:

FOR THE PART OF HAIR FALL: It is quite common to find air fall in men which is known as "male pattern baldness" is most common cause of hair fall in young men like you.

This process of falling starts by 18-20 years and is highest in between the age of 20 to 30 years. So it is quite obvious that you can have the starting of this problem of hair fall just because of hormonal influence.

Other than male pattern baldness there can be falling of hair all over the scalp can occur after simple stress too !!!.

Nevertheless in hair fall poor diet contribute to increase the hair fall. SO you should eat healthiest of diet.

For the betterment of diet kindly follow the suggestions below:

1. Eat at least 3-4 different fruits saplings per day. Every fruit is good but you can make a combination of sour with sweet fruits such as oranges, papaya and other of seasonal fruits.

2. Pulses are good source of proteins. So everyday have a small bowel full of mixed pulses. Add green leafy vegetables additionally in the pulses which contains lot of iron and vitamins in it.

3. Sprouts are the best source of fibers, minerals especially iron, vitamins and proteins too. so add it at-least alternate day if you can not make in every-day's part of meal.

4. Over night soaked and peeled almonds will give you good vitamin E which is good for the hair growth.

5. Milk and milk products should be included in every-day's schedule.

6.Do light exercises, reduce your work stress as all these have bit impact on skin so also on scalp skin.


FOR seborrheic dermatitis with DANDRUFF AND HAIR ROUTINE:

Scientifically and medically our skin is a multiple layered structure. Our skin gets renewed every day. The layer which is present on the top today will be fallen off tomorrow. It will not be present tomorrow and would be dead skin in form of scales. This process is not actually visible on normally. So we can say we are just one day old from our skin most superficially.

Now in some persons it is renewed at a faster pace. Because of hygiene and scalp hair this dead skin in form of scales gets entangled and trapped in form of visible of scales known as dandruff. It varies from person to person and also on other of factors which makes the difference inter-individually.

Coming on to your problem which is scientifically known as “pityriasis capitis” is highest in twenties and then recedes by itself in later part of life. It is due to the fact that the renewal of skin and the fatty acid secretion in scalp is highest at this part of life. So there is a tendency to grow fungus as well which makes the flaking more perceptible. There are no such important predisposing factors except for few which can make the situation worsened such as individual's own tendency to renew the skin, application of hair oil and environmental factors.

We can not change our own tendency to renew the skin but the personal measures of hygiene as well as the environmental factors can be modified.

For the remedy part follow the things below:

1.Wash your scalp daily. I guide my patients to wash twice in a single bath. It means when you are washing your scalp apply first time shampoo for washing the hair part only, then rinse it off immediately with water. Reapply the shampoo again immediately, leave it for around 5 minutes, do a good gentle massage by your fingers for around 2 minutes and then wash it off. By doing so for around 7 days you will get good benefit.

2. Stick to one good shampoo which is suiting your hair, don’t change the shampoos frequently. If you are using any conditioner also then to be used over the strands of hair only and not over the scalp skin. There is no difference of shampoos, the only part is to use good company shampoos such as Laureal or Tresemme shampoo. IF the dandruff is too high then you can use any medicated shampoo containing Ketoconazole in it.

3.Do not use any kind of hair oil or any oily or creamy stuff any time on your scalp.

4.Comb your hair at least 3-4 times a day and most importantly at night time so that the dust particle and other of dead skin is taken off.So for betterment of high oil secretion the only good solution is daily washing with any good shampoo. Reduce your exposure to hot and humid climate with any consumption of spicy food.

You can follow the above regimen for 8-12 weeks periods. This is my personal suggestion as a dermatologist that rather than medicines, simple daily regimen works better than anything.

Regarding ozone treatment : I have not found anything permanently better with this treatment so I leave the decision on your side for the same.

Hi Sir,

Thank you very much for your prompt reply, as you have noticed that being young and having hairloss can have significant impact on individual's self-confidence. The same crisis I am going through as well. I have noticed that specially during seasonal change my condition is worsened specially with onset of winter.

I am definitely going to follow the diet regimen as recommended by you and would contact you again in future for further guidance.

But some questions I still have in my mind:-

-How propasalyc NF lotion is able to relieve me? May I have any other skin disorder other than as mentioned by you like psoriasis, as it works like a magic for me stopping dandruff and hairfall completely, right from its very first use.
and whether it is possible to use an intelligent schedule for its use to sustain its effect.

-Hair wash makes my hairs dry, rendering them very ugly(as if I never wash them). Do you think sebamed shampoo is worth being used on daily basis?(the shampoo advertises itself to be ph-balancing as well)

-How to solve my acne issue, I have extremely oily scalp and skin both specially around my nose. As I have mentioned earlier too, isotretinoin makes my dandruff & hairfall worse.

-I am said to be a person who is introvert and thinks a lot(specially depressing one), does it also aggravate my problem?

-Shall I avoid sugary diets as well, including tea and coffie?

-And this is last one from my long list of queries, do you think in future I mean in my 30s, my problem will automatically slow down, so is it a good idea to keep them as short as possible for the time being?(in my 20s) :)

Regarding your further queries:

-     Propysalic NF lotion is basically a steroid based lotion. It is a property of the steroids to suppress the inflammation and flaking. Therefore in your case you find dramatic response. It is better to taper off it slowly. It means to apply on alternate days and then one two times per week before completely stopping it.

-     In my patients I use a gently shampoo such as Sebowash for initially few weeks and then use 8X or Xgain shampoo later on. You can try and use Sebamed accordingly in place of Sebowash Shampoo.

-     Acne is a recurrent youth problem and gets better with age. Though Isotretinoin is one of the very good treatments but if it is not suiting then can discuss other options from your treating dermatologist.

-     Stress and over thinking definitely increases pimples as well as dandruff problem. So I suggest you to keep yourself stress free.

-     Diet has no direct implication in either pimples or dandruff. But more of sugary things can sometimes aggravate eh condition in little percentage of patients. You can just observe if you have more eruptions after taking sugary things. If such thing happens then can avoid the same.

-     Yes, definitely with progress of age the hormones get more settled. So you would have less of upsurge of hormones and accordingly fewer pimples and less dandruff issues.
