Suggest Treatment For Sever Pain On The Knee

Question: I have pain on the outside of my knee and believe it to be a ligament issue as it does not hurt a great deal when I walk on a flat surface . I walk 4 kms daily
Pain experience walking up and down stairs. Walking dow stairs is more painful than going up Am unable to lay on my right side for long without experiencing pain that interferes with my sleep
I had both hips replaced 18 months ago, which I had 5 years too late. Until then the pain from the arthritis was severe and I wouldn't have known if I had this problem then as there was so much transferred pain from the hip joint

Brief Answer:
sounds like ligament injury
Detailed Answer:
Hi,
I am Dr Mittal.
I have read your query.
I think I would agree with your opinion on the subject.
I feel the symptoms and the way it's present are quite suggestive of a ligament injury too. However, an alternative would be a bone issue that's in healing.
The best way to diagnose this is by an arthroscopic evaluation or an Mri of the affected area.
I hope that this is the information you need. Please feel free to contact us for more information. Best of luck. Dr Mittal.

Dr,
Assuming it is a ligament injury, and the fact I have had it for at at least 6 months possibly longer, what could the actual problem? Why hasn't it changed in condition over the longer period. I have actually given the walking a break for several weeks to rest it but there is no change.
I would have thought this type of injury pretty common.......is that the case, and if so what treatment would you suggest. I take Padadol Osteo for my shoulder, but it has no effect on my knee.

Needs mri

Hi,
I have read your response.
And it's a good question.
The fact remains that you have been on physiotherapy for the hip condition. Maybe that movement was avoiding the healing.
However, in the lack of actual examination, I would still suggest MRI of the knee to be sure of the diagnosis.
I have tried to make it as simple as possible. I hope that the information helps you. Please feel free to contact us for more information.
Best of luck. Dr Mittal.

Sir I appreciate your responses, but i think you should be able to tell me from past experience , as a GP, what the actual problem may be................all you have agreed with me is that it may be a ligament problem. Which ligament could it be, how many ligaments are there in the knee? If its as you believe, whats the treatment
If it is, what do you actually suggest may be the damage that's typically experienced in these cases, what treatment may be required to rectify it.?
Can it be arthritis based upon the symptoms I have suggested
Look forward to your response

Detailed Answer:
Dear XXXXXXX
You have to understand that when I see a patient in a clinic, I can examine them and that tells me a lot of things.
A condition can have many mimics.
The possibility of this being a ligament injury is as strong as it being a bony injury that's healing.
If it's a ligament injury that we talked about, we will need to assess the level of repair that the body has achieved on its own. I think the physiotherapy you were on may not have allowed it to heal completely. I would suggest a rest to the knee with a high protein diet. An MRI is needed to visualize the extent of repair. It may need surgical repair if not healed. After 6 months, if there is no healing, it's unlikely to repair on its own. If on the other hand healing has occurred but it still needs time, then we will have to use rest, a knee brace and high protein diet with cold fomentation.
If just based on the history I would imagine that the probability that there may have been a chip of bone that came out and that is healing slowly would need more time to heal. Besides an X ray may be needed to see if it's healing at all. If it's not healing, you may need surgery.
I have considered both the options. As you may have understood, we need to know what's going on inside the body. A clinical judgement and use of experience tells me these are the 2 possibilities we have to consider.
