Thanks for highlighting the facts about the illness of your fiance.
She had severe UTI with stone in the kidney .
Ideal line of treatment in such case is to do urine culture to trace out the organisms causing UTI and to give appropriate antibiotic as per culture report and once the infection is under control then to do ESWL or Stenting.
Since she has landed in an accidental injury to her ankle and had toundergo surgery to fix it . I would suggest youto follow folloeing line of further treatment for kidney stone.
1)Please consult qualified Urologist for clinical assessment and get her routine urine test and urine culture done to find out the organisms causing this infection and antibiotics to which they are sensitive to.
2) Repeat urine culture after 2 weeks and if the infection is well controlled you may go for second sitting of ESWL.
3) She may need multiple sitting of ESWL till stone gets fragmented completely..
Once the stone is fragmented completely and she passes all the stone fragments and gets complete stone cleared as revealed by X-ray or Scanning .You may remove her stent

In a severe case of Septicemia blood culture will reveal the nature of bacteria and their sensitivity pattern .
However it is always advisable to do urine culture in cases of Urosepsis as it is more specific and will give number of colonies of organisms in urine .
