Suggest Treatment For Severe Abdominal Pain In An Elderly Person

Question: 51 year old female presented with c/o Abdominal pain since 3 days with not passing stool nor flatus with bloating of abdomen with nausea and 3-4 episode of vomiting.

Patient took glycerine suppository after which passed black coloured stool.

Diagnosed to have Adhesive (band) obstruction.

Underwent exploratory laparotomy with adhesiolysis .

Patient is having h/o Hysterectomy 10 month back.

Is the current diagnosis is related to previously done surgery of Hysterectomy?

Detailed Answer:
Hi.
Thanks for your query, very specific.
Noted the history and gone through all the papers and report you have posted.
Patient had a history of chronic constipation for 3 years and Hysterectomy done in March 2016 that is one year back.
Developed symptoms for 3 days before surgery.
Surgical noted say: Adhesiolysis- Division of band.
Since there is history of constipation for 3 years - Hysterectomy 1 year back but the development of obstruction only for 3 days it will be difficult to say whether the problem was long standing as per history of constipation of 3 years, it is difficult to say whether this problem was already present for 3 years or was due to Hysterectomy as a known sequel/complication of any intra-abdominal surgery.
If Surgery has to cause intestinal obstruction it would usually develop within 3 to 6 weeks of surgery as the scars start getting matured.
Your operating Surgeon can tell you the best as the observation-on-table indicate towards the cause.
