Question: Hi this is XXXXXXX i am suffering from severe acid reflux from 10 days . Mychest is paining and burning all the time . When i take TUMS it is cool for2 hours and same problem kepps on hurting me ..

So the thing is i started a diet to loose my weight on 18th april i continued for 10 days i found some thing os wrng with my body so i started etting again from 28th april (rice,curry) so tody is 5th of may so what can i doo its still hurting mee my heart is buring . Is this the gastric problm or somethng else going on .. iam worried can you help me please

Hello,

Have gone through your details and i appreciate your concerns.
Initially, I would like to say that you are not treating acid peptic disorder properly that is why you are not relieved.Basic cause of your problem is reflux of acid from stomach to esophagus.Stomach is lined by a natural layer of mucosa membrane which is resistant to acid however mucosa in esophagus is easily destroyed by acid which regurgitates back from stomach.This cause severe irritation of esophagus leading to inflammation causing heartburn chest pain etc etc.Presentation varies from patient to patient.
Now managemebt consists of two step-
1)Control acid production- PPI's are available in market for the same.

2)Prevent reflux of acid formed back to esophagus-Take prokinetic drug for the same.

Please request your gastroenterologist to start you on PPI-antacids like Rabeprazole or Esomeprazole.In case you can't get a prescription omeprazole is OTC.Take 40mg twice daily.However best is esomeprazole.Esomeprazole can be used in the dosage as high as 80 mg twice daily.Trials have shown that esomeprazole is superior to other PPI's in controlling reflux symptoms. Also, request him/her to add Domperidone 30mg or Levosulpiride(both are prokinetic)slow release once daily. This will slow down the reflux of acid back.Prokineic should be added in your regimen.But you would need a prescription for it.

An antacid containing local anesthetic (Mucaine gel ) should be taken 2tsf thrice daily.Just taling TUMS won't help.

Acotiamide is another wonderful new drug and is very effective in controlling symptoms of GERD and esophagitis. In trials- Acotiamide, a gastrointestinal motility modulator, at a standard dose of 100mg thrice daily has significantly affected esophageal motor functions or gastroesophageal reflux in healthy adults.Not available in USA get it online from Europe.
To prevent the esophageal damage from developing into Barett esophagus we would follow aggressive treatment pattern initially. As your symptoms will improve drugs can be tappered off.

Try Eating more fermented foods. These are rich in both good bacteria and enzymes you can try raw natto kefir or cultured veggies. This is probably one of the most important first steps.
Take a high-quality probiotic.
Take external enzyme supplements.


Follow up after 10 days of taking medicines.

Iam 25 iam from XXXXXXX XXXXXXX lives in californiaa .. i feel like weight near my heart .. i could not get any medicines.. so can you tell me how many days does ot take to cure completely . Soo my diet effected my system ??? .. so what should i eat .. can i take curd buttermilk a lot

You need to take proper medication and diet as suggested and you would start feeling better in 3 days.
First lets control your symptoms after that we should think of cure.
Cure would take time life style changes as well as patience.
Your diet anxiety levels overlooking symptoms improper medication all are responsible for worsening of your state.
You could take chilled milk butter milk and curd as much as you want very good for you.
General precautions are-
Follow a bland diet (low acid levels, low in fat, not spicy)
Eat frequent, small meals.
Lose weight.
Avoid the use of alcohol, tobacco, and caffeine.
Do not eat food less than 2 hours before bedtime.
Raise the head of the bed before sleeping.
