default
Posted on Mon, 12 Dec 2016
Question: Hi,
10 days ago I had loose motions, which stay fit 2 days and went to bathroom over 20 times during it.
I took some antibiotics and it goes after 2days.
But since then I am experiencing pain in anus during regular potty. It is severe pain and sometimes blood is visible on the stool or/and tissue paper used to wipe rectum.

Please suggest what causes this and the medicine I need to take.

Thanks

Brief Answer:
you have anal fissure

Detailed Answer:
Hello,
1) Your previous episode of diarrhea resolution with antibiotics suggest it might be due to non-specific intestinal infection (enteritis). You should have done stool examination for routine and culture. If your stool consistency and stool frequency are becoming normal that suggest resolution of infection and so not do stool tests now.
2) Pain around anus during passing stool + bleeding on wiping anus suggest possibility of anal fissure and possible internal hemorrhoid. You should consult general surgeon for digital rectal examination and proctoscopic examination.
3) In anal fissure, there will be linear breach on inner layer of lower part of anal canal which is responsible for occasional bleeding and pain. In acute case of fissure, sometimes you might develop inflammation (redness and swelling) also which subside with treatment over the time.
4) Medical treatment will help to relieve pain and heal by reducing swelling. Constipation is major reason for development of fissure. Thus, treatment for constipation is must and also never strain much to pass stool as this increase chance of bleeding and pain.

Thanks doctor,
After the diarrhea the Stoll consistency and frequency become normal, it's being 8-9 days already.
I generally don't have constipation and even now it is very normal.
So don't think I still need the stool test.
Based on your suggestion, it is surely the anus fissure.

Can you suggest some ointment for the inflammation and redness since it do inflame for few hours after the Stoll.

Thanks

Brief Answer:
No stool test.

Detailed Answer:
Hello,
1) No need of stool examination now.
2) For ointment I suggest you to consult general surgeon as content of ointment may vary depending on stage of anal fissure.
3) Generally anesthetic agent ointment will help to relieve pain but it will have no action on swelling.
