Suggest Treatment For Severe Bleeding During Pregnancy

Question: Dear Doc,
Would request your kind advice on my wife's situation. She is 11 weeks pregnant(1st pregnancy ended in missed abortion in 7 weeks in 2013) and has been diagnosed with sub-chorionic haematoma. She was hospitalized on 15th Feb 2015, due to severe bleeding. Upon hospitalization and subsequent USG, the same was detected. Post some days of medication, her bleeding have stopped. But, her Hb % had reduced to 6.8% even after a bottle of Orofer FCM infusion in blood. Taking no risk, to her and the developing baby, she was put on 2 pints of packed RBC. Post 24 hours of transfusion she is feeling better. Today, another sonography was done which detected that the clot have increased 5 times from 22 ml to 120 ml. The fetus is ok with normal heartbeat. We are extremely tensed, specially looking into the fact that further bleeding might make her more anemic. Please advice whether the treatment that is being followed is in right order and how would you suggest to improve further for safety of mother & baby.

Treatment Prior to Hospitalization
--------------------------------------
1. Thyronorm 50 mcg
2. Duphaston 10 mg (Twice Daily)
3. Folic 123 (Once Daily)
4. Ecosprin 75 mg. (Once a day)
5. Fertygin 5000 (Once a week injection)
6. Susten 200 (Alternate Day IM Injection)

Treatment during Hospitalization (15th Feb onwards...contn)
----------------------------------
1. Tranexa (Injection twice daily to stop bleeding) (Stopped wef 21st Feb)
2. Monocef (Injection twice daily) (Changed to Tablet wef 22nd Feb)
3. Duphaston 10 mg twice daily
4. SeaCure AQ (Injection once a day IM)
5. Pan 40 Tab.
6. Folic 123 Tab.

Mam, please advice on the efficacy of the treatment and how you you advice to improve it further.

1) This bleeding is called threatened miscarriage.

2) You are on right treatment and medications. There is nothing more that can be added to this list of medicines.

3) I suggest you and your wife be relaxed and dont think about negative aspect much. Because worry and stress itself can be a reason for miscarriage.

4) This treatment is efficient and your wife is likely to carryon pregnancy with good fetus. Continue present treatment.

Dear Sir,

Appreciate your kind advice.

I am hereby attaching the 2 USG done post hospitalization. First was done on 16th Fen and the second was done today (i.e 24th Feb 2015). Our today's scan reveals huge increase in the size of the haematoma. Would request you to kindly advice on such huge increase in the clot, even though she was under complete bed-rest at the hospital. Also kindly share the risks to the mother, as she is already down on Hb%.

Looking forward to your kind advice and any other info which would help us to treat better.

Blood clots will resolve

Hello,

I saw ultrasound scan report attached.

There is definitely increase in size of hematoma.

But this does not mean end of hope. There are many instances of such pregnancies safely reaching to term. These clots will slowly resolve.

You are on right treatment as of now. Blood transfusion will take care of hemoglobin issue.
