
Suggest Treatment For Severe Bloating And Burping With Gastric Discomfort

I have been having severe bloating and burping for fiyr no the with upper gastric discomfort. I had a mildly ekevated CA19 but doctors not concened. They did an MRI of abdomen abd pancreas which was normal but an ultrasound yesterday found a bulky and heterogenous pancreas. I was tested for pancreatitihs but all bloods were normal so they ruled that out. What else could cause my symptoms and a bulky pancreas? Could it be an early pancreatic cancer that wasn't seen on MRI three months ago? My other synptoms are easy bruising and fatigue and eye floaters

I read your query and understood your concerns.
After reviewing all your data I would like to say that it's a great thing that all blood tests (for pancreatitis) and your MRI results were normal.
I would like to know the exact value of CA19-9 in order to give a more accurate opinion. If it's highly elevated I would like to suggest endoscopic ultrasound which is the best examinations for the evaluation of pancreas.
Moreover other causes should be blamed for your symptoms and findings including:
- GERD
- stomach issues
If you were my patient I would recommend to do an upper endoscopy in order to exlude acid reflux (GERD) or other stomach issues.
If the upper endoscopy is normal than, based on the MRI and abdominal ultrasound data, my opinion is that you most probably have fatty pancreas problems.
Please keep in mind that abdominal MRI is more accurate (compared with conventional abdominal ulttasound) and is the examinations of choice for evaluation of pancreas cancer. This is why it's almost impossible that MRI would have missed pancreatic cancer.

i have had a gastroscopy and ruled out any gerd or other stomach issues so I am concerned that on the ultrasoubd she found a bulky pancreas with heterogenous appearance. Is that how a fatty pancreas would appear on us?
I know MRI can miss tumours that are small and I'm wondering now that 4 months have passed since that was done perhaps I need someone to actually look inside pancreas and see what's going on?

Let me explain you again about pancreatic cancer:
Pancreatic cancer is suspected open elevated CA19-9. The reference range of serum CA 19-9 is fewer than 37 U/mL. The CA19-9 should be between 37-100 U/mL in order to suspect pancreatic cancer.
However in case with highly susceptibility for pancreatic cancer we recommend doing abdominal MRI as in your case (it didn't found any pancreatic mass). Since the XXXXXXX CA19-9 results were normal the probability to have pancreatic cancer is very low and most likely the ultrasound findings are related to fatty pancreas.
I would recommend repeating again CA19-9. If it will be above 37 U/mL than the susceptibility for pancreatic cancer will be high. If these will be your case (since your MRI was normal) the next examinations to find pancreatic cancer (as I mentioned above) is endoscopic ultrasound (EUS). Tumors are more accurately detected - or excluded - by EUS as compared to CT or MRI.

Although the CA19-9 level is increase to 31 U/mL, it's still below 37 U/mL and you shouldn't worry about anything.
However in order to definitely exclude any doubt I strongly recommend to schedule for an endoscopic ultrasound (EUS). EUS will help to diagnose even small pancreatic lesions that even MRI can't find it out.
