Welcome To HCM. I am Dr Anshul Varshney. I understand your concern.
You seem to be suffering from Acid reflux disease associated with Irritable bowel syndrome (IBS). Since your scans are clear so gall bladder issues are unlikely. In IBS you have variable presentation of stools and a lot many people get sand like stools as you have described.
In my opinion an upped GI Endoscopy should be performed to rule our Peptic ulcers and Gastro esophageal reflux disease. Also stool for occult blood should be performed to rule out any Upper gastrointestinal bleed.
You should also get an ECG to rule out any possibility of Cardiac issues that can cause chest pain.
I would recommend in such scenario to take a combination of Pantoprazole and Domperidone which helps greatly with reflux.
In case you have any associated weight loss or anemia you would also require evaluation of Malabsorption Syndrome like Celiac disease. If your doctor suspects that in Endoscopy he will order a blood test known as Anti TTG Antibody.
