Could you be able to help me,please XXXXXXX
You need to get a chest X Ray and PFT done in the first place
Detailed Answer:
Dear XXXXXXX
I am surprised that you are having cough for last 30 years and you haven't yet sought proper medical help and only relying on some herbal treatment and cough expectorant (please let me know which specialty of doctor/s have you visited and what all management have been done so far).
I would like to know if you are suffering from any allergies and are you a smoker?
Do you also have breathing difficulty or do you wheeze ?
Are you taking any BP medicine esp FROM ACE inhibitor group (eg. drugs like captopril, enalapril, lisinopril, ramipril etc.) ?
You should first get a chest X Ray and a PFT done.
Since you have cough for so many years, I presume it could be allergic cough. You may take a tablet of MONTAIR-LC (MONTELUKAST-LEVOCETIRIZINE) daily and use Symbicort inhaler (Budesonide/Formoterol) 2 puffs daily [you may need prescription from your local doctor for them]. If you are a smoker please stop smoking.
Do answer my questions so that I can explain you further about your cough.
Take care.
Continue with inhalers and other antiallergic medicines.
Detailed Answer:
Since you have already been to all the concerned specialitists, i presume that things like GERD and chronic sinusitis have been ruled out. You must have also got HRCT of chest done to rule out ILD(Interstitial Lung Disease). These are common causes of chronic cough.
In my practice i have come across a few cases where an unusually long uvula(the soft hanging portion in the mouth) was the cause of this cough as it was irritating the throat. Though it is rare. You may consult an ENT specialist and discuss this with him. Also i hope you do not have any heart ailment, otherwise do consult a cardiologist.
At this stage i can only suggest that continue with Montair LC and Symbicort. Be regular with them because many a times allergic cough is not controlled as patient is not taking inhalers regularly.
Take care.
