Suggest Treatment For Severe Depression And Anxiety Due To Physical Pain

Question: hello, i had a stroke year ago last dec. wasnt found out until Nov after seeing an eye doctor. It blew a hole in my heart which was patched in XXXXXXX I now have 5 tendons torn in my right shoulder with onn retracted giving me a "popeye" arm. Doctors dont want to fix it cause they say my arm is "functional". My left shoulder has 4 torn tendons, these all started to hurt the night I actually had the stroke. I wantn them operated on and fixed. I have been on oxycodone for over a year, it has been a blessing for the pain, also feel liike knives are stuck through my back on both shoulkder blades. Have had 4 arm operations since stroke for compressed nerves, now have sever depression, moderate anxiety and pain mind problems associated with physco something and general pysical condition. Seeing a pyschologist. Never been sick in my 60 years. I am allergic to codiens and hydrocodone which is all surgeons in the houston areas will prescribe for post surge, my PM will not treat after surg pain, I';m stuck out, i dont want to suffer more pain than I already have, cut on and left medicating myself, at 60? Not a good idea. U know any surgeeon that can understandn my problem and will fiix shoulkders and have inough compasssion to help the post surrg pain, or should I just wait for God to heal me, thanks

Thanks for posting you query.
After going through your suffering, you need physical examination to rule out the cause of your pain. Do you have any injury to both shoulders that may have caused the tendon injury? Are you a diabetic? Please share.
The main symptoms of a complete rotator cuff tear are weakness with or without pain. You may do certain test at home which may rule out/confirm the rotator cuff tear-
Hold your arm straight out to your side (90 degrees) with your thumb down. Lower your arm slowly. If it drops suddenly, you probably have a rotator cuff tear.
Please share the MRI report if already done for both shoulder, if not you may get it done to confirm the diagnosis.
Psychosomatic symptoms are exacerbated with depression and anxiety. So, you should continue counselling and medication as advised by psychiatrist.
You may have the following test done after discussion with your doctor-
Injecting an anaesthetic into your shoulder may relieve pain but will not restore strength if you have a torn rotator cuff.

Based on the above discussion, my advice would be having faith on your treating orthopaedic surgeon and you may take analgesic along with physiotherapy to reduce the pain for next 3 week. I hope you will get better with it and your pain management will continue as before without putting it at risk.

This is quite painful, break through pain even with oxycod 7.5-325apap, can not find surgeon who will treat me with post op meds that not allergic to, I feel im on my own on this, non of this had pain until the stroke, i think the pain made my BP go up so high it caused the stroke, partial blind now in left eye after hole blown out in heart. thank you, I will travel to get this done if my insurance is accepted and if surgeon is smart enought to take care of post op pain without hyd
codone which I am highly allergic to, I throw it up 15', horrible med. for me,thanks

It’s good to see you
I have gone through the MRI report and whole discussion we had, based on that my observation is you have complain of pain and pain only and there is no functional impairment like difficulty in self care (coming of hair, eating food, dress up). Am I correct? The diagnostic feature of rotator cuff tear is inability to lift arm and loss of shoulder movement with or without pain.
Having said we have to treat the patient and not the MRI films. These MRI reports are old one and the lesion might have healed of its own.
The shoulder joint’s are sensitive joint and take 9-12 months to return to normal in cases with minor injury/frozen shoulder with different phases of pain only then restriction of movement only and so on. So I feel you might be going through the pain cycle of shoulder suffering.
In conclusion, my advice would be –
A.     Continue with pain management and physiotherapy team to reduce your suffering for another 3-4 weeks. If you feel better well and good. Otherwise-
B.     Get fresh MRI done and discuss with the arthroscopic surgeon in your city. To control the post operative pain we may use Ibuprofen, Naproxen as you are allergic to codeines and hydrocodone.
If for some reason arthroscopic surgery is not possible you may use medical tourism facilities to visit other country like India, Singapore or some other country of your choice for that matter.

Thank you Doctor. This pain is far beyond Ibroprophen, I also have the knife felling in both my shoulkder blades as well as trapped nerves. I do not sleep well and find I have to get up at 3-4 am, I am tired and tired of pain. The pain actually breaks thru Oxycodone and I do not complain as I once mentioned it and the doctor said "just what I thought, you have become tolerant to the medication". I leaned to tough it out after that because I figure some pain relief is better than none at all. The mental issue is a result of a tragic accident I was responsible for 36 years ago. I knew I have suffered depression for years but after the pain it was like putting a pea on a full plate and the items on the plate started to fall off. I only started seeking help after I was "found" out having been ordered by my neurologist ordering that I take a neropysch evaluation, busted! Because of the accident that was not viewed as such in court I have lived in a show for years. Being confident on the outside for people and dying on inside, not afraid of dying. I know I cant undo what happened but only wish I could trade places, I would gladly do so without a second thought. I know this has nothing to do with my shoulders or arms but i hope gives you a better insight to my total condition. After the stroke I can not wiggles my fingers on right hand well at all, I am considered severly impaired on my right hand with atrophy, my forearm has gotten skinnier, I have lost 54 pounds in 6 months. No appitite and the pain in my shoulders is horrible, between pain and bad memories, guilt it's pretty tough at times,ot crying about it but telling you the truth. I do thank you for your time very much, thanks XXXXXXX Only problems other than memory and tears is dressing when shoulder pain is at XXXXXXX and lifting anything with either arm. Dropping toothbrush, cutting meat, small things like that

I have my sympathy with the suffering you are going through; besides taking pain killer you definitely need surgical intervention as you are not able to do activity of daily living.

After the long discussion we had let us focus on remedy for your shoulder pain, my advice would be-

-Get fresh MRI for your shoulder and take a second or third opinion from shoulder surgeon in Houston or at other city in XXXXXXX Discuss with the pain management team for post operative pain.

-The other option we have already discussed earlier. Meanwhile continue with the same medication along with physiotherapy. You may have cortisone injection inside the joint (intra articular) and medicine like pregablin, under medical guidance. This drugs helps in relieving pain in arms following stroke and neuropathic pain.

I have been turned away from those sugeons (4) that either do not understand my allergies to ceetain medicines, I have consulted my pain management doctor, she will continue with my pain medicine but said boldy, she will not treat post surgical pain, i understand because in case the surgery goes wrong she does not want to be in the loop, attys, not at my request would drag her into court as part of the package. I have also thought about new MRI's on both shoulders as they have gotted even more painful and at time my right hand shakes. sometimes my balance is bad, standing up or whike standing sometimes i take four or five steps backwards and often I walk as tho I'm drunk, my right legs wants to go in front of my left and I srtart walking to to right at a 45 degrees, Why I am stuck out is the agreement I have with my trusting pain XXXXXXX doc. I can not bring myself to find street drugs stonger then the oxy, which I guess would be full strength heroin. Thats exactly what I was trying to protect a loved one from when the shooting accident happened, thru negligence it wasnt the heroin that killed them it was me thru stupid neligence when the gun discharged i had to protect myself from the guy who carried a pistol and was providing her the heroin, It would make me a huge hypocrite and break me blessed trust my pain doctor has in me.
That the guilt I have been carrying fir 36 years, like dragging a body behind me no matter where I go, I only wish it were me that was in the path of an old 410 shotgun. I suffer that I want to see her again but know I am not to hurry that. I know you cant diagnose over the internet without seeing me but I think my physical pain is greater than you can understand with examining me, I understand yet appreciate the direction you give me and your time. I have come to the knowledge that I have a better chance of waiting for God to heal me in his time and/or wait until he tells me it is time I go to be with him in any matter. I will ask for a third MRI as with me neck a third MRI has shown it has gotten worse, now I have 2 of the four ruptured disk impenging the cord, trapping the nerves. I'm not certain if you are Indian or nor, but I seek those from india because of the intelligence and compassion they have with knowledge. those who have no concept of med allergies were not indians, they were morons, yet I forgiven them. I will ask my indian pain doc to order another MRI if my shoulders and will go from there. I will let you know what the MRI's now show. I wish there was a different way of discussing these things with you or at the least send you copies of the MRI's so to make it more clear to you just what I'm facing, thank you, you have spent more time then most doctoirs I have been to, thank you! Those who do arthroscopic surgery will not, they say my right arm has to be "fileted" in order to sew my bicept back together, they turn me away.
