Suggest Treatment For Severe Dyspnea And Lower Back Pain

default
Posted on Fri, 5 May 2017
Question: Rheumatology question: I have severe dyspnea from toxic mold exposures, and have been testing positive on a lot of autoimmune marker labs recently. I also have severe low back pain of unknown origin, with recent urine analyses and CT imaging coming back normal. Could any of the following lab results be considered problematically high in an autoimmune condition sense, whether individually or collectively?

Myeloperoxidase antibodies: 1.9; H (< 1.0 = no antibody detected)
Proteinase-3 antibodies: 1.9; H ((< 1.0 = no antibody detected)
Complement, Total (CH50): > 60 U/mL; H (standard range 31-60 U/mL)
DNA (DS) antibody: 5 IU/mL; H (< or = 4 negative; 5-9 indeterminate; 10 = positive)
Chromatin AB: 1.1 IU/mL; POS AI (< 1.0 NEG AI)
ANA CHOICE ABS CASCADING RFX: positive (standard range: negative)
Forgot to mention other symptoms: hyperactivity/agitation; difficulty falling or staying asleep (I sleep about 5 hours night); hypotension (bp commonly 80/60 and can be lower); low body temps (95-96 F basal thermometer oral reads); left hip pain; TMJ (if you put index finger in opening of left ear canal you can feel ear pop forcefully in and out of joint whenever I open or close my jaw). Please let me know you received additional symptom list ^. This should be a premium question, and I would anticipate giving you five star review value for informational value you give me.

Brief Answer:
Possibility of autoimmune disease is less

Detailed Answer:
Hello
the antibodies to pr3 and mpo are positive.
However these are indicative of severe systemic vasculit is.
These diseases are associated with fever.sinusitis, lung involvement in the form of cavities and pneumonia.
The inflammatory markers like esr and crp are also raised in this condition.
However these features do not seem to be present in your case.
and the symptoms have also been going on for a while.
so maybe it is a feature of your allergic condition.

Are you saying that with severe systemic vasculitis conditions which pr3 and mpo antibodies are characteristic of (e.g. Wegener's granulomatosis with polyangiitis) always present also with elevated esr and crp values? Esr and crp (both heart crp and endothelial crp) labs have so far always been normal.

Yes with Wegener's disease there is pulmonary and sometimes renal involvement. I have had severe dyspnea since 6/1/2016 from toxic mold exposures, and spirometry data in 11/2016 my medical chart notes say is indicative of severely obstructive airway pattern (FEV1/FVC 0.55, FEV1 50% predicted). CT imaging of lungs has not shown abnormality. I also have had severe low back pain, but urine analyses have come back normal and CT imaging has not shown kidney stones or other problems.

Other symptoms:hyperactivity/agitation; difficulty falling or staying asleep; hypotension (bp commonly 80/60 and can be lower); low body temps (95-96 F basal thermometer oral reads); left hip pain; low back pain; general body pain; TMJ (if you put index finger in opening of left ear canal you can feel ear pop forcefully in and out of joint whenever I open or close my jaw).

I am living in a mold-contaminated environment, and have high IgE antibodies to mold. Do you surmise based on the above provided info I am dealing with allergy and not Wegener's disease or other vasculitis or autoimmune conditions?

Brief Answer:
Yes unlikely to be vasculitis

Detailed Answer:
Hello again,

Chronic obstructive airway disease is unlikely due to Wegener's.

It is a serious disease and as your problem has not been treated with systemic immunosuppressants, without which patients can get very sick.

So I feel this is more allergic with other somatic symptoms.

Regards
