Question: my husband (who never has a headache) is experiencing a severe headache that worsens when he leans forward. We just got off work in a retail environment where he was standing all day (except for a brief lunch sitting down)

Hello dear,
I have gone through your question and understand your question.
Severe headache unbearable in this age group can be due to stroke, subarachnoid hemorrhage. In some cases migraine headache can occur with exertion , meal missing in person with previous history or propensity.
If your husband is not having vomiting, visual blurring, diplopia, facial deviation, limb weakness, drowsiness, seizure or loss of consciousness then the possibility of danger headache is less.
One possibility of subarachnoid headache can be there even in absence of all these features with presentation with very severe headache.
Migraine headache is usually associated with throbbing feeling, nausea, photophobia, phonophobia.
In light of previous diseases and medications it will be better to consult a doctor if not relieved by paracetamol and rest.
