Suggest Treatment For Severe Headache, Throwing Up Of Blood, Skull Pain And Numbness
My wife has heavy headache from last few months, we consulted neurologist, he advised sebilium 10 mg for a month. During taking this tablet, she was fine but from last few weeks she is complaining heavy headache. We consulted a general physician for any infection, he advised antibiotic for 3 days, now she is fine with headache. But from last 2 days she vomiting little blood in the morning during brush time and also her appetite got reduced, she not taking food properly. She complains more of tingling effects in head, numbness, skull pain, right side heavy headache and throat discomfort. We have done a plain CT SCAN of brain which was normal. I am worried because she is not getting well. Now from last 2 days I see little blood while spitting in the morning during brush time. But otherwise whole day she is fine and there is no blood while spitting and her headache is under control. Kindly find the attached blood reports. Please suggest if any other tests if need to be performed. Kindly help us in this regard.

The two most common causes of Headache are Migraine Headache and Tension type headache.
The blood in spit seems to be unrelated, but can be due to any pain killers if she is taking.
I need following details to help her better:
1. The headache is persistent or intermittent?
2. Any throbbing, nausea, vomiting associated?
3. Headache is on one side of head or both the sides?
4. Is Sibellium giving her relief?
5. How is her sleep?
6. The blood is from mouth or in the cough?
7. Any fever?
8. Any other medicines she is taking?
9. Any associated coughing?

A : Its intermittent. mostly she complains during evening time.
2. Any throbbing, nausea, vomiting associated?
A : No. No nausea, vomiting, some throat discomfort she has.
3. Headache is on one side of head or both the sides?
A: Her headache is mostly on right side of head.
4. Is Sibellium giving her relief?
A: Yes, when she took it for a month she was fine as prescribed by Doctor. After stopping this medicine her headache increased.
5. How is her sleep?
A : She sleeps but not sound sleep. Sometimes she gets up in between sleep and complains headache,numbness right side of her head.
6. The blood is from mouth or in the cough?
A: No Cough. Blood is only during morning brush time from mouth.
7. Any fever?
A: Her temperature was 99.5 few weeks back, Doctor said its light fever and she took dolo-650/77216?iL=true" >dolo 650 for 2 days as prescribed by doctor along with anti-biotic
8. Any other medicines she is taking?
A: after done with sibelium, she took dolo-650 and anti-biotic for 3 days. Currently she is not taking any medication.
9. Any associated coughing?
A: No she has no cough

Migraine headache
Detailed Answer:
Hello, Her description given by you is suggestive of Migraine type headache only.
You should get her a Chest X-ray and Complete blood count to rule out the various causes like Infection for the issue of blood in the spit.
Also seek a dentist opinion to rule out any evidence of oral cavity or dental issues giving rise to bleeding.
Now, for Migraine prophylaxis most commonly used drugs like Flunarizine (Sibellium), Propanolol, Amitryptilline.
In case of acute pain Triptans like Sumitriptan, Rizatripans are used. In case of acute pain not responding to Triptans, we use pain killer NSAIDs like Diclofenac.
She also needs to modify her life style. A person suffering from migraine should sleep at least 8 hours in night, at fixed hours.
One should avoid sleeping in day time, as it causes difficulty in falling asleep on time in night and the whole cycle of peaceful sleep gets disturbed.
Avoid alcohol specially beer, caffeine, lemon, cheese in diet.
She needs to follow these lifestyle modifications for the purpose of getting rid of medications after a few months course. People who modify there lifestyles, usually get rid of medications after 6 months course.
