I read your question and i understand your concern. Unilateral eye dilation with history of migraine triggered by exercise needs to be clinically correlated and thoroughly evaluated for migraine variants and other possibilities of neurological causes. Migraine has many variants, its possible that ophthalmoplegic migraine may present like this. Treatment is same, supportive, abortive, prophylactic measures all should be tried . Avoid triggers, keep migraine diary, try botox injection for the relief of migraine. COmplete neurological examination and eye examination is mandatory to rule out other pathologies.
