Question: patient is having severe pain, 9-10 intensity. been going on for 3 days has a lump with indentation on left wrist with pain radiating to shoulder. No hx of gout, but took Allopurinol about 10 yrs ago because of elevated UA. Wants to know if she should go to ER, as she has already taken 7 reg. Tylenol and 2 Tylenol Arthritis strength plus 2-3 rx for itching with pain., (just today). She has tried multiple times of heat/cold therapy, and it doesn't help. I told her go to er, but they have no money. please advise to YYYY@YYYY . Thank you

Hello -

With pain that intense, yes she should go either to an urgent care clinic or ER to address the pain.

She can also try an NSAID such as ibuprofen (with food so it doesn't irritate her stomach), but if her pain is 9-10 in intensity, that may not be enough to help.

I can't tell without examining her what the lump in her wrist is. My guess is that if it isn't apparent what it is, they will want to get an X-ray of the wrist in the ER.
