Question: I am suffering form Partial Supraspinatous tendon tear, from around 8 days, its quite painful, even on taking the analgesics in form of tablets pain is not getting relieved, my eyes get open due to severe pain in night. What should I do? Is surgery option? or it can be managed conservatively, how to decide that? Hot fomentation helps me in relieving pain, I am a regular user of laptop, can operating a laptop also puts strain on this muscle, which shoulder support should I Wear? taking oral steroids will help in improving the inflammation ? I never take steroids but this time bcz of pain I am thinking this way.

Brief Answer:
Do not take steroids

Detailed Answer:
Hello,

I have studied your case and I have lot of experience with such cases and I would suggest you to wait for some more time may be a month. Some partial tear get healed in time and we can avoid surgery.

Till that time you can take following treatment.
1. Tablet Etoricoxib after discussing with your doctor.
2. Short wave dithermy to shoulder applied by physiotherapist. This is better then hot water fomentation.

If your pain continues then You should get this tear repaired. It is done through Arthroscopy and it is not a very big surgery.

Laptop working can aggravate pain as it needs your shoulder to be in same position for long time and it can cause fatigue and pain. But it does not increase size of the tear.

Shoulder support you need is called as universal shoulder immobilizer, it has to be used when you are going out so that your tear should not progress.

Oral steroids should not be taken as it can increase tear and delay healing. Steroids are not a option. If pain is not controlled by drugs then treatment is surgery. Steroid will also cause more chances of infection if you plan for surgery.

I hope this answer will be useful for you. Let me know if there is any other followup questions. I will try my best to help you.

Thanks
