Question: I have been having severe pain in my legs (primarily right). Strong hot streaks of searing pain from my knees down to my feet. My feet hurt as well. Some pain from my hips to my knee on the right. So bad it is difficult to stand and walk. My knee doesn't hurt, I can bend them and put weight on them. I do have back pain (chronic spinal problems since 1988)

The pain in the knees and leg that you have can be related to the spondylottic changes in lumbar spine, osteoarthritis, low levels of calcium or vitamin D3 or a muscle spasm.
Taking pain killers in combination with muscle relaxants can help with the problem. Do get your blood levels evaluated for calcium and vitamin D3 levels. You can start with the supplements according to the report. Continue doing physiotherapy exercises regularly for a long term relief.

What do i need to do to get a diagnosis? Blood test, xray, etc? Do i go to my GP or to a specialist? My lumbar injury is covered under workers compensation and i see a pain dictor. I use muscle relaxers and pain medications already. What is my next step?

You need to consult an orthopedician and get a few blood tests and an MRI scan done to find the proper cause for this. You can use pain medications and muscle relaxants in addition to local analgesic gels and a hot compress. Regular physiotherapy may also be needed.
