
Suggest Treatment For Severe Pain In The Leg After A Fall

Question: Dear Dr,
I want your advice on my dad case.
My dad is 85 years old who fell over 2 days ago. He has severe pain and cannot mobilizes his right leg or walk.
Would you please advised me what do you think from radiographs attached?
He suffers from prostate cancer, COPD and a controlled high blood pressure.
He smokes 5-10 a days for last 50 years.
He does not drink alcohol.
He only takes Combodart for prostate enlargement.
He started taking diclofenac and paracetamol as pain killer since he fell two days ago.
What are the treatment options for him and what is most likely his treatment will be?
What is in your opinion is the best treatment for him taking consideration his age.
What do you think if just leave him un-treated (bed rest)?

I am posting this again to find an unbiased answer to my dad death.
He dies at home 9-10 days after the operation:
1. The operation went well and was in good health after the operation (just tired).
2. The surgeon discharged him after 3 days following the operation, saying everything is ok.
3. His health getting worse and started having asthma like symptoms.
4. A local heart specialist saw him 5 days after discharge and advised to have oxygen machine at home which done.
5. He died after 7 days of discharge.
Please give me some detail about what exactly happened and why he died?

I am very sorry by reading this ,I just can not believe this.It is very difficult to give exact cause of his death, as I told you earlier surgery at this stage and age was very risky.Some of major cause is blood loose ,WHICH CAN BE DECIDE by pre and post operative Hb report. 2. Deep vein thrombosis.3.Fat embolism.4. Post operative respiration complication.5.Other and exact cause can be decided only by autopsy.One thing I want to personally say to you that leave all the discussion it has no use except to make you puzzled and disturbed.May his soul rest in peace.

Was it guaranteed to to save his life without complications ?

Difficult to decide the cause of death.
Detailed Answer:
The conservative treatment- traction would have reduced the blood loss and all the complications caused by the surgery. The traction would have increased the recovery time and could have caused chronic complications like malunion or bed sores and inability to walk again but would have saved his life. It could also have deteriorated his quality of life and made him bed ridden. Boot and bar increases the life expectancy but may deteriorate the quality of life. Surgery causes an instant recovery but since your father had so many other problems also, was difficult to accomplish and get a good result.
There was no guarantee of saving your father's life in either of the treatments as his general condition was not good. He might have died due to COPD as he was tired and had difficulty in respiration for which oxygen was given. The stress of surgery or fracture can aggravate COPD.
