Suggest Treatment For Severe Pain In The Leg

Question: Hi Doctor,

My Mother aged 64 years is having severe pain in leg. We have visited Vascular surgeon Doctor in XXXXXXX who have prescribed for physiotherapy and medicines. I took my Mother to Vascular surgeon thinking that the symptoms seem to be like vercose vain problem.

Once the tablet and physiotherapy is discontinued the pain starts coming in the leg. Could you let me know what could be issue and what should be treatment followed and which specialist should I see to solve this pain in leg.

Hi,

Thank you for posting the question.
I have gone through the medical history and the attached reports in detail.

It seems that the pain that your mother is experiencing in her legs is not due to varicose veins or any vascular problem.

This pain seems to be arising out of a pinched nerve or nerve inflammation.
This can be due to neuritis or a slipped disc in her back.

She needs to get a MRI of the lumboscaral spine done along with Electromyography and Nerve conduction studies of the lower limb.

Depending upon these reports you can consult either a orthpaedic surgeon or a spine specialist.
Give her anti-inflammatory tablets with methylcobalamin and pregabalin tablets till then.

The greenish blue enlarged veins in her leg may be due to early varicose veins due to prolonged standing episodes but this is not responsible for her pain.

Wearing stockinettes around the leg will be helpful.

Hi Doctor,

Thanks for the detail reply. I have few clarifications.

1. My mother is complaining of heaviness of leg after prolonged standing or walking that subsequently develops into pain.....Is the root cause is DVT ?

2. The doppler showed negative results for varicose vein - Does this mean the varicose vein is ruled out.

3. This pain seems to be arising out of a pinched nerve or nerve inflammation.
- What will be recommended treatment ? will it require surgery or physiotherapy to solve the pain?

4. Can we take the MRI of the lumboscaral spine done along with Electromyography and Nerve conduction studies of the lower limb before visiting doctor or is it recommended to take after visiting the doctor with your details?

Planning to visit the Doctor based on your feedback as the pain is severe that my mother is not able to sleep. The pain aggravates while sleeping and the physiotherapist is asking to keep the leg on a set of pillows.

Hi,

DVT and varicose veins is completely ruled out once you get the doppler done. You already have done it and that should be enough to rule out DVT and varicose veins.

For a pinched nerve,physiotherapy with traction,Short wave diathermy,LASER therapy are the options.Surgery is required in extreme unresponsive cases.

I suggest you to visist your doctor first and then get a MRI and EMG/MCV studies done.
