Question: extreme pain on right side just below the rib cage and unable to have bowel movement.
what might be causing the pain?

Hello,
I can understand your concern on GI related symptoms.
1) Pain on right side of abdomen + unable to have bowel movement suggest possibility of severe constipation or hard stool.
2) Constipation cause inadequate defecation which lead to collection of stool in large intestine and this cause dilatation of large intestine.
3) Dilated large intestine has sluggish movement which further aggravates collection of stool/undigested product and thus vicious cycle continues.
4) For constipation you should continue to take stool softener or laxative. You might require enema or stimulant suppository during such acute episodes of constipation. Occasionally you can also take anti-spasmodic group of medication (Tab Spasmoproxyvon) to control abdominal pain associated with intestinal distension. I would like to mention here to take this medicine only in intolerable pain as it also make intestinal movement more sluggish.

1) this is colon area from large intestine traverse from right to left and then move downwards towards anus.
