Suggest Treatment For Severe Pain In The Shoulders

Question: I feel pain in left shoulder joint. Initially it was very low intensity but slowly it is increasing.
I did not get any treatment. Please suggest.

Shoulder pain may be caused due to wide variety of reasons including:poor posture,frozen shoulder(due to stiffness of structures around shoulder)any injury to the soft tissue structures around the shoulder (known as rotator cuff), arthritis of the shoulder joint (usually older age group), shoulder instability(usually younger age group).Occasionally it might not be related to shoulder (due to compression of the nerve in the neck) or can be due to injury or inflammation of other bones and muscles around shoulder joint.We need more information to diagnose the source of your pain like: any injury, duration of pain, any response to medications, any injury in the past, your age, any other illness like diabetes (can be associated with frozen shoulder).etc.If you provide these details it would be easy for us to narrow down the root cause specifically.

The treatment varies depending on the spectrum of the symptoms and includes rest, ice, analgesics (oral tablets or topical sprays/gels),physiotherapy(exercises and modality based like ultrasonic massage and short wave diathermy) help initially.

Pain which is showing an upward trend (increasing with time)needs evaluation by an orthopaedician for early relief.

My shoulder pain might be caused due to poor posture during night sleep. Because I felt pain in the morning before that night i had no pain.
So please suggest me what to do now.

Hi Mr XXXXXXX Thanks for your follow up question.

At your age , with no history of injury,if you are able to move your shoulder siimilar to normal shoulder: I think we can wait upto 4 to 6 weeks (most pain subsides in first 2 weeks) before we consider any tests or intervention.

Meanwhile, I would suggest you to take simple medicines for pain relief, such as ibuprofen or paracetamol(dose appropriate to your body weight -can be given over the counter by pharmacist in India).Use topical gel or sprays two to three times daily (can also be taken over the counter ).Can apply Ice packs to reduce pain during first 3 to 4 days. I suggest you to avoid activities that may cause more pain in your affected shoulder, like lifting any kind of wieght or sleeping on the painful side.If you have access to physiotherapy, ultrasonic massge and short wave diathermy can help in earlier recovery of pain. I would suggest you to not to any shoulder exercises till pain subsides for a week.I am sure this should help you get relief from your pain.

However, you should personally see an orthopedic surgeon if pain gets very bad(in spite of medications and gel), does not resolve within a 4 to 6 weeks or associated with fever or aggravated by an injury. Depending on the cause of your shoulder pain, further treatments may be needed.
