Suggest Treatment For Severe Pain On The Shoulders
![default](/r/images/default.png)
Question: Hello My mother is 58 yrs old. She woke up 2 days ago with pain in her right shoulder. She describes it as a continuous dull pain that radiates down the arm. Initially she thought it started from behind her shoulder, but now she feels its starts from the side of the shoulder/deltoid, while being more painful around the elbow. It can radiate all the way to her hand. There was no known preceding injury. The pain occurs mainly when she hangs her arm by her side, and she finds some relief using a home made sling. She is able to move her arms fully in all directions. She does not feel any weakness. She does not have any tenderness in the shoulder, arm or elbow. She has no neck pain either. She is normally well apart from being hypothyroid. I am a GP, and I tried the tests for all 4 rotator cuff, as well as hawkins for impingement and these were all negative. 2 days on pregabalin in case it was nerve root pain hasn't so far reduced the pain either. I was wondering if the info so far is able to give a suggestion as to what is going on. Thank you for your help.
Brief Answer:
Adhesive capsulitis
Detailed Answer:
Hi there
Thanks for the query
Usually hypothyroidism is associated with Adhesive capsulitis or Frozen shoulder. Early stages may not demonstrate typical signs but dull aching pain that can radiate to the elbow.
Physiotherapy with SWD(Short Wave Diathermy) and range of movements of the shoulder will be beneficial. Application of warm packs or fomentation will also be beneficial.
An ultrasound of the shoulder can reveal a connected shoulder capsule in such a case.
I hope I have answered your query.
I will be available to answer your follow up queries.

Thank you
Organising a US scan may take some week on the NHS. When Should I ask my mother to begin physio exercises for frozen shoulder. Is early as possible , safe or should I let pain settle first.?Is there advantage in doing it early?

Brief Answer:
Let the pain settle. Get the ultrasound confirmation.
Detailed Answer:
She can start taking NSAIDs and once the pain is settled, she can start physiotherapy. Let her start once the ultrasound scan is done. Till then just Short wave diathermy (SWD) and hot fomentation should suffice, with normal shoulder movements including lifting the arm above the shoulder level.
