Question: My wife has severe plantopalmar keratoderma. Acitretin tablets, and UV-light therapy have not helped. I have read that fluorouracil-5%-topical- cream ("Efudex" and "Carac") has been very effective in some cases, and so I would like to buy some for her to try. However, at the moment I cannot find a UK supplier (I am a registered UK veterinarian, and could write a UK prescription for it) My email address is : YYYY@YYYY

I have gone through your query. Palmoplantar keratoderma can be very restricting and I totally empathize with you and your wife.

Treatment of palmoplantar keratoderma is challenging and the options available provide only symptomatic and short term improvement.

However Oral retinoids i.e Acitretin is the most effective of all and usually produces good results. It is the only available oral treatment.

I would like to know what dose of acitretin that she takes? and whether she has tried topical Vitamin D analogues like calcipotriol? Can you upload a few Images of the skin involvement? Is the involvement diffuse or localized?

Apart from Oral Retinoids other available treatment options that may be tried include Topical keratolytics like salicylic acid 6% or 12% ointment; topical tretinoin; topical Vitamin D analogues; topical 5-fluorouracil 5%.

Response is variable.
