Suggest Treatment For Severe Polyhydramnios

Question: Hi,
I'm suffering from severe polyhydramnios.
As such, would like to know whether drinking water can actually aggravate the symptoms of polyhydramnios?

I understand your concern..

Drinking water has very less role in polyhydramnios.. So there is no point in decreasing the fluid intake.. Excessive intake can be avoided.. 1-2 litres per day can be taken..

Uncontrolled diabetes can present with severe polyhydramnios.. so get a glucose challenge test to detect diabetes.. Or a FBS and PPBS to rule out diabetes..

Also a scan is necessary to rule out ant fetal anomalies which is causing polyhydramnios..

Hi Dr Dailia,

Thank you for your answers to my query above.

I have another question which I'm in doubt about treating polyhydramnios that I'm currently suffering. I'm at 29 weeks, going on to 30 weeks then. Also, I had already done the test on GD (Gestational Diabetes) and the results are had been negative.

I've been recommended to do amniocentesis and to drain out or reduce the fluid as an option. But apparently, was told that this procedure shall have to be done repeatedly as it could only last only for 2-3 days and the fluid could return thereafter.

Also I've been informed that this procedure might not be safe as it would cause the rupture of my uterus, leakage of fluid and there would be a risk of infection as well.

Please confirm on the above and are there any safer methods in treating the condition?

I understand your concern..

What your doctor informed about polyhydramnios is absolutely right.. Drainage of fluid reduces the overdistension and symptoms associated with it.. But it has all the complication which was informed..

Another medical management is indomethacin tab taken orally.. But fetal echogram should be repeated every 3 weeks due to fetal cardiac problems associated with it.. It cannot be taken beyond 34 weeks..

Scan has to repeated 2 weekly to assess fetal well-being and fluid level..

Hi Dr Dailia,

Can I know whether it is necessary to take additional zinc or calcium & magnesium supplements during my pregnancy.

Currently, I'm on obimin tablets but it doesn't contain any zinc or magnesium inside it.

If yes, how much is the recommended dosage for zinc, calcium and magnesium per day for pregnant mothers at the 3rd trimester.

Normally only folic acid, iron and calcium is prescribed in pregnancy.. In pregnancy tablets are not usually given for zinc or magnesium supplementation..

So you have include lot of veggies in your diet which will naturally provide those minerals..

I am concerned and would like to clarify whether the polyhydramnios condition could lead to the occurence of preterm labor.
If yes, what are the risks or is there any complications to the baby if it was prematurely delivered at around 30 weeks due to the above condition? Or at 31 -32 weeks? Does the baby required to be put in an incubator at 32 weeks or later?

What is considered the safest time frame for a baby to be delivered and not have any complications at all?

Don't worry dear.. Actually what ever you asked is absolutely right but I don't want you to worry at this point of time since mother's tension can affect the fetus..

Polyhydramnios can cause early rupture of membranes and often preterm labour.. So complete bed rest is recommended.. You can take 2 doses if betnesol for fetal lung maturation after consulting your doctor.. It helps in early maturation of lung.. After 34 weeks there won't be much problem to the baby.. prior to that incubator might be required.. Don't worry about all these.. your doctor will be taken care of it..

Hi Dr Dahlia,

Since I'm having the problem of polyhydramnios, I have difficulty taking much food and my food intake is limited due to stomach tightness.

Hence, is it possible I rely on liquid meals like milk, oats or cereals only instead of having heavy full course meals.

Also, will I be nutrient deficient or lacking if I'm unable to eat a variety of nutrient rich food during the 3rd trimester then.

You can take small amount of food at 2hr interval which is nutrient rich.. A huge meal need not be consumed.. It is good to include oats., ragi and liquid diet.. In between include veggies, fruits, milk, egg and nuts which provides adequate nutrients..

In the 3rd trimester the above meal which is nutrient rich is adequate for good fetal growth.. So no need to worry..

Hi Dr Dahlia,

Just a question to clarify on the the expected delivery date (EDD) which I'm told based on the pregnancy due date calculator.

Why is it that the no. of weeks for each pregnancy is supposed to last till 40 weeks instead of 36 weeks (9 mths x 4 wks)?

Do I have to wait for the pregnancy it to last till 40 weeks then for a safe delivery instead of 36 weeks? Is delivery at 36 weeks also considered full term then?

My doctor had examined me and informed that the baby is suspected of having the condition called esophegeal altresia (which is the cause of the polyhydramnios).

Is this a serious condition and what are the risks to the baby upon delivery?

I understand your concern.. Esophageal Artesia can be treated surgically after birth and prognosis is good after the surgery.. So don't worry..

The extent of Artesia need to detected which can be done only after delivery.. Based on that immediate surgery our surgery on a later date is planned..
