Suggest Treatment For Sharp Back Pain

Question: I have a history of fractured vertebrae in my lumbar and cervical spine. While on vacation, my lumbar spine started hurting me with sharp pain. Movement increases it. Vacation was one week ago. I cannot take anything with ibuprofen as I had a major DVT surgery on January 9th and have been put on Xarelto and Asprin 325 every day. My vascular surgeon said I would most likely taking them the rest of my life. At this point, my back is at the point where I cannot bend over, if I sit, I have to have help standing. I cannot turn or reach for things without severe pain. I have tried Flexeril and extra strength Tylenol without any relief. Can I use a heating pad or should I go with ice? I know I need to have X-rays but haven't got a way to get there at this time.
