Suggest Treatment For Sharp Pain In Back

Question: HI I'm 19 years old and I injured my back in practice a couple years ago in Basketball practice running and I've never been the same. I have a strong achey sharp pain when I lie down for a few seconds and when I bend over and stand up straight the pain is terrible and nothing helps it. I went to a orthopedist and he took xrays and bone scans and a mri and nothing showed up. He just told me I had a week core but I'm 6ft 170 and I definitely don't have a weak core, so I did core exercises and physical therapy for 6 months and it didn't help, so then I went back and he took more xrays and said nothing is wrong and told me to do more core exercises which didn't even help It makes no sense why I hurt this much and he says nothing is wrong. I've been going to a chiropractor for the last few weeks and he says I have a "subluxation" in my back which is weird because the orthopedist didn't mention that. But the chiropractor is barely helping and it's expensive also. I also hurt my hip flexor the year before and my right hip snaps every time I lift my leg. And my left hamstring bothers me and I hurt that in practice too, when I straighten my leg out to stretch or even walk up stairs it feels like this one tendon is rubbing against another but it doesn't pop it kinda twitches kind of like it's being pulled apart. Basketball is my life but it's also hurt me pretty bad and I want to play college ball next year, any answer will be awesome. Thank you so much!

I have gone through your question in detail and understand your concern.

You are having chronic backache. Your x ray, bone scan and MRI are normal. You are not getting benefits from Physiotherpy. I think you should get your vitamin D3 levels and if found low you can take its supplements. For pain relief analgesic such as ibuprofen are of help. Back excercises are helpful.

Thanks for answering doctor. Exercise doesn't help if makes it worse and so does stretches. Do you think I should keep going to the chiropractor or stop?

Till you have pain you should give rest to your back and stop excercises. You can restart back exercises after you are relatively pain free to strengthen your back muscles. You can gradually increase excercises to your tolerance and if pain increase with excercises then these should be decreased or stopped. I think you should stop going to chiropractor.Rest already has been discussed.

Sorry for asking so many questions Dr. But I've suffered from this so long and I need answers. So I did take a couple months off from all activity because the other doctor said I had a bone fracture so I quit everything and it still hurt then I went back and he gave me physical therapy and it didn't help so I actually rested for a couple months and did physical therapy after that but still a no progress, then he told me there was no fracture so I have no idea.. Would vitamin d really help my pain go away? And why does stretching make it worse? Thanks Dr!

I have gone through your question and follow up questions and understand your concerns. Vitamin D3 deficiency is important cause of backache and unexplained muscular pains. So you can get its levels and if found low you can take vitamin D3 supplements and it will be beneficial. You have said that your x ray,bone scan and MRI are normal. Much older fractures (older than 1 year) are unlikely to be cause of pain. If painful muscles will be stretched these will cause more pain. One more idia came in my mind that thyroid disease should be excluded by thyroid function test(T3,T 4,TSH). Rest already discussed.
