Suggest Treatment For Sharp Pain In The Abdomen

Question: Hi, recently was in the ER with sharp pain in right. My blood work was good and urine had no blood but the pain lasted a few hours. Thought it could be a kidney stone but they did not see anything on the ultrasound. The left kidney showed a 2cm lesion that my pcp would like a better look at. He first ordered an MRI with contrast and changed it to a CT abdomen & pelvis with contrast. Is this the type of scan I should be getting? Any risks with the IV contrast?

Thanks for your query ,based on the facts that you have posted it appears that you had severe pain in abdomen and had been to ER and routine blood work and urine test were reported to be normal and Ultrasound Scanning of of abdomen did not reveal any stione but revealed lesion 0f a size of 2 cm in in Right kidney .

The lesion in the kidney is strongly suspicious of a mass in the kidney and has to be confirmed by plain and Contrast C.T of abdomen and Pelvis which your Primary Care Physician has rightly asked for .

Now a days they use Non Ionic Radio Opaque Contrast XXXXXXX while doing any radiological investigation.This contrast medium has no incidence or risk of severe allergic or anaphylaxis reaction.

Please go ahead and get it done as it will help us to highlight the nature of the lesion .

Hi,


Thank you. Just wondering is a Ct of abdomen/ pelvis with contrast the best to see this or is there another type of scan or area a specialist would order? Only asking because I don't want to have to get re scanned, and my pcp first ordered a MRI then changed it. I have heard other people mention a uroscan? With and without contrast? Not sure if that's the right word but it was something like that.

We Urologist always to do Plain and I.V Contrast CT scan of abdomen and pelvis that covers entire urinary system and organs in the abdominal cavity .It gives precise diagnosis of any lesion in kidney along with infiltration if any .

Ok, great. So I should get it with and without contrast? Is it a lot more radiation? And what would the advantages be without the contrast?

Contrast C.T Gives precise information about the infiltration of the lesion ,nature of the lesion (whether benign or malignant ) and its effects on surrounding tissues which is a deciding factor in modality of treatment ..
where as plain CT Scan will detect the lesion and its site and dimensions .
