The shortness of breath and cough can either be a primary issue related to the respiratory tract or can be secondary to the gastrointestinal pathology. If secondary then it can be due to the aspirated food content. Difficulty swallowing can be due to esophageal pathology or pharynx dysfunction. This would be more evident on the endoscopy. Drinking too much fluid can be a symptom of a raised blood sugar. Diabetes workup including fasting blood sugar and HBA1c levels should be done. If the shortness of breath persists then oxygen saturation needs to be checked for especially if there is an associated altered mental state or light head / dizziness complaint. You can take him to an ER where passing a Nasogastric tube for feeding purposes can be considered for nutritional purposes if there is considerable difficulty in feeding.
