Suggest Treatment For Sinus Infection Of Fungal Etiology

Question: Getting pain in both eyes like pain coming from back side /inner of eye and of eye balls, - itchy, swelling on /above eye lids- some like yellow/pus like stuff in bottom of eyes inside
does such condition due to severe sinus infections-and need emergency consult,
you can see from sinus pictures, where all the four sinus are infected.
what do you suggest while doing endoscopic sinus surgery -sphenoid sinus also should be operate- or ENT surgeon will keep this as ,you suggest.
does eye infection due to sinus infections-
penicilliun sps infection is high

I am Dr. DADAPEER K, an Ophthalmologist and I answer health problems related to eye.

I reviewed your history and the reports of the CT scan attached with the question.

It seems from the reports that you are having sinus infection of fungal etiology.
The infections of the sinus cavities can spread to the eye and the orbit.
Hence the sinus infection needs to be treated as early as possible.
The fungal infections are usually resistant to treatment and hence sinus surgeries may be required as in your case advised by your doctor.


Regarding operating for sphenoid sinuses, this decision needs to be taken by your operating ENT doctor depending on the presence of infection and its severity.

Only use of ketorolac eye drops will not help to prevent infection, hence I would prefer to use antibiotic and antifungal eye drops as you are having yellow pus like discharge from the bottom of the eyelids.

as far as eyes are concerned - can infection/fungal/allergy can be tested by taking flux or some touch based from eyes.
you are sure that these are fungal infection in all sinuses.
here super trained doctors are not able to accept - from the pictures of CT and allergy tests for fungal.
What do you think that while waiting for surgery for like 40 days from now would be safe or it can be serious alarm,

The infection in the eyes can be confirmed by microbiological tests in the form of culture for the underlying bacteria or fungi.

Hence I advice you to go for microbiological tests to diagnose the causative agent and if the diagnosis comes as fungal then you need to go for early surgery.

can you mention details of the both CT scans for sphenoid- and all seriousness, word copies of CT are also attached please.
do you noticed any difference in word copies and actual pictures, how far Sphenoid is infected?

Thank you for the follow up question.

I went through the reports attached with the question.

The CT scan done on 25-11-2014 shows involvement of all the four sinuses and thickening of the mucosa of the sinuses.
The follow up CT scan done on 23-12-2014 shows improvement and the sphenoid sinus is not involved in this scan.

The word copies are in consistent with the CT scan pictures.


Hence the report shows improvement as on 23-12-2014. Since it is almost 2 months from last CT scan we need to repeat this to know the current status.
