I have gone through your query; and suggest you that first try the mucolytics such as acetyl cysteine 200 mg five times a day. Also, you may have fexofenadine & montelukast once a day with your ongoing treatment after consulting with your ENT specialist for at least three days.
If the symptoms improve then continue the conservative management otherwise consult with your ENT specialist for surgery.
But to rule out other disease and to reach the near diagnosis you may go for the impedance audiometry.
Information regarding the surgery related with your condition are grommets are tiny plastic tubes inserted into your ear drum in a short operation. Grommets may be recommended if you have glue ear that won't clear up, or frequent ear infections. It allows air into the space behind the ear drum (middle ear) which reduces the risk of fluid building up there.

I have gone through your follow up query and suggest you that you can send the image of your ear drum by sending the link after upload in Google drive. For clear image it is better to go for the otoendoscopy.
If you have a ruptured ear drum then in above mentioned treatment you may add
1. Antibiotic ear drop two drops three times a day.
2. Avoid putting water, oil and ear bud in the ear.
