Suggest Treatment For Skin Infection
Question: Hi,
I had a skin infection around 10 years back near my inner thighs and genital area. First I used dettol which caused some burn and then I consulted doctor who prescribed me cream and powder which I used. But my infection did not go away properly and it kept coming back and there was darkening of skin near that area. I used same kind of anti infection cream whenever I felt some infection after consulting doctor.
Now my infection is under control though i easily get dry skin on that area and easily get rashes there but due to long lasting infection, sweating, not properly cleaning of my inner thigh area regularly, friction (rubbing) and other matter the skin condition on both side of inner thighs doesnt look very good now. I will try to give the picture for your reference.
Let me know if there is anything can be done to improve the skin condition especially the apearance as it is very embarassing?

Hi. Thank you. I have reviewed the Images.
I can see that you have dry, scaly patches in groin folds along with darkening of skin. It seems like the fungal infection is still active.
Hyperpigmentation/ darkening is part of post-inflammatory hyperpigmentation.
If I was the treating doctor I would suggest a topical antifungal e.g Clotrin-B Or Candidi- B cream (combination of clotrimazole 1% + beclomethasone dipropionate 0.025% cream). You may use it twice daily for 2-4 weeks.
Orally I would suggest tablet fluconazole once every week for 4-6 weeks.
Once the infection resolves (scaling resolves), thereafter you must continue using an antifungal dusting powder regularly in groin folds. This would keep the folds dry and also prevent fungus from proliferating again.
Darkness of thighs would resolve gradually provided recurrence of fungal infection is prevented.
These are prescription medications and must be used under supervision and guidance of a dermatologist and are to be used only for the prescribed duration. Therefore I suggest that you also visit a dermatologist in your vicinity for a confirmatory diagnosis and for the needful.

But just to let you know that I do not have any constant itching or any discomfert feeling in this area. Is it normal to have active infection without constant itching?
Also is there any medicated soap or body wash I should use during shower which can help?
I am also worried of using anti fungle cream again and again and it may damage skin as I have used many times in past but the problem is not going?

Itching is a subjective symptom. Some patients may itch constantly whereas others may not have a constant itch. They itch, but it is tolerable.
The fact that you have itching, even if it is not constant, indicates that there is still some active fungal infection. Moreover, scaling is a sign of activity.
Fungal infection may return after cure, if conditions conducive for fungal proliferation tend to prevail therefore an important part of antifungal treatment is to prevent it from recurring. Therefore after completing a full course of Oral and topical antifungals I usually advice my patients to use an antifungal dusting powder in groin folds for maintenance of improvement. Powder will keep groin folds dry and is also inhibitory to fungal growth.
Antifungals creams are safe if they are used under supervision of a dermatologist and for the duration prescribed by doctor. Unsupervised use and certain over the counter antifungal creams may damage skin specially because of the steroid content that is present in these creams.
Antifungal soaps can be used and are available e.g candid soap Or Sastid bar but according to me an antifungal dusting powder is better suited for maintenance.
