After reading history and seeing the photos you posted, I think, most probably you may be having chronic folliculitis. Folliculitis is a very common, benign skin disorder that looks like scattered pinpoint red bumps on the skin.
Folliculitis is an inflammatory condition affecting the hair follicles. It appears as a small localized area of pus surrounding the opening of a hair follicle. Older lesions that have lost the pus appear as red bumps surrounding the opening of the follicle. One to hundreds of follicles can be affected anywhere that hair is present. You have lesions on buttock.
Most common cause is stahylococci bacteria for the folliculitis. There may be other causes like fungi,viruses..etc.Most cases of folliculitis are completely curable. There are very uncommon, long-standing cases of folliculitis that may not be curable. Often these more resistant cases may be controlled with proper treatment and medication. Folliculitis sometimes clears completely by itself without treatment. In your case, the folliculitis is chronic and relapsing. So, it might improve after long term treatment.
I usually advise treatment after thorough investigations in my patients. Diseases like DM,Anaemia,HIV and other internal disease may be ruled out.
Oral antibiotics like cloxacillin, cephalexin or cefexime may be considered for few weeks. You may take courses of antibiotics at intervals of few weeks.
Topical application of antibiotics like sisomycine or mupirocin may applied. Sitz bath in tub containing water mixed with dettol daily for 10 min.
Improve immunity taking immunoboosters. Take vitamin C and antioxidants for long time.
Have patience for the good result.
Consult dermatologist and have regular and proper treatment under his observation.
