I understood your health concerns of your blocked hard lumpy vein with no flow in it.
-Could it be prevented-
Yes,but this is with high risks and cost with heparin therapy.
Its prevention is left to natural process of body scavenging system which clears such events in the body.
Thrombophlebitis is a natural occurrence at the infusing sites,and is due to the thrombotic response of the vein wall to the infusion trauma to the vein.
Extent of the this response varies from person to person,upon the bodies reactivity.
For Timely Prevention-Timely Treatment and Care needed-
What is needed is the start of the treatment to control this Thrombophlebitis,when one gets it out of proportion.If treated the flow could be maintained and regained in 2-3 mths time and may be 6 mths time in few cases,in early stages only

In your case the thrombophlebitis seems to have blocked the vein by the resultant blood clot, which got fibrosed over a time,which is a natural outcome of the resolved blood clot in the vein.

-Resolution of the block is possible by use of the tPA(Tissue Plasminogen Activator)like repletase/urkinase,which is currently used in arterial and DVT blocks by infusion.
-A new technique(PCDT-Pharmacomechanical Catheter Directed Thrombolysis) of clot busting by direct tPA injection in to clot can dissolve the clot and could re-establish the vein flow.
But as the cost and risks of this treatment outway the benefits,this is not commonly used in superficial vein thrombosed veins.
-Any cause of worry for DVT/lung embrocation, with this Vein clot ?
Being superficial vein clot of DVT /or lung embolisation is very remote.unless this risk is impending,there is no point in planning any treatment in your case.

I am sorry that my last reply was confusion. Let me try and clear this confusion.
Part of my reply is to educate you on the possibilities of the available treatments which are only needed if you have troubles from the thrombosed right arm.
At this point of time, I don't see any serious issues from the thrombosed vein yet. The only issue was blocking which generally gets revascularised over a period of time. If the blood flow doesn't return to this vein, body has abilities to create new channels to establish blood flow in this region. Therefore I wouldn't actively treat this condition at this point.
Treatment options with tPA or PCDT is necessary if there is a complication which is extremely unlikely from this blocked vein.
That being said, if you are worried and apprehensive about this problem, you can consider the conservative methods.
In short. if I were your treating doctor, I wouldn't actively treat this problem yet. But if you insist on a treat I would chose to treat it with those conservative measures.
