Question: Hello Doctor, I am 34 yrs old female. I had a knee scratch with little bleeding yesterday. As I was out so just places simple bandage on it. water was leaking out of it y'day. Today I cleaned the wound with dettol and left it opened. But its burning all the time and paining too. Also feeling light fever. Suggest any painkiller or tablet to prevent from infection. and if any cream is required for wound. Shall I keep it open or covered? Thanks

From the description that you have provided, it seems to be a minor scrape wound over the knee surface. Since the specific area is always at motion while walking and sitting, so is the reason for its delayed healing. It will heal by itself in a period of 2-3 days.

I would suggest you to do the following:
- wash it with dettol daily and keep the wound open.
- applying bandage over small wounds will cause moisture retention.
- watch for the signs of infection- increase in swelling, redness over the area, discharge of yellow pus.
- take Tab. Lyser- D twice daily for 2 days and SOS.

Nothing to worry.

Hi Doctor, Yeah i can see yellow pus type on it. Is the tablet for pain or infection? As Ii am feeling restless due to pain. Can I take some paracetamol painkiller?

Since you have mentioned that you can see yellow pus from the wound, I would suggest you to start a course of antibiotics.
- Take antibiotic containing Amoxicillin and clavulinic acid combination (available as brand name AUGMENTIN) 625mg twice daily for 5 days.
- The medicine i had previously mentioned LYSER-D is anti- inflammatory and analgesic, will help in healing pain and inflammation.
- Also if the wound was caused by falling or scraping some sharp object, I would suggest you to get an Inj. Tetanus Toxioid.

It was due to falling and scratched from sand. Trying to attach again. I will wait till tomorrow to take antibiotics if the yellow discharge does not go. I am starting with Lyser-D now. Thanks

I have seen the attached picture.

The wound looks like it has started to heal partly, though the discharge is not clear enough to distinguish between a pus or normal serous discharge seen during the process of healing.

Anyways I would suggest it best for you to take a course of antibiotics and an inj. TT, so as to avoid any further complications.

I would suggest you to do the following:

- like I said before, look out for signs of infection: increase in discharge, discharge stained with blood, foul smell of the discharge, increase in pain and sweling.

- If you find any of the above symptoms, it is better to start with the antibiotics as soon as possible.

Hi,
Thanks for the follow up,

I was able to view the recent pictures you have attached.

Yes, it looks like the wound is infected.

Kindly do the following:
- daily cleaning and dressing from a nearby hospital.

- application of topical mupirocin ointment, which is a topical antibiotic.

- keep the area clean and dry.

- take tab. Lyser- D twice daily for 5 days

- also start on oral antibiotics Tab. Cefadroxil 200mg BD for 5 days, this is a broad spectrum antibiotic which will cover most of the infective bacteria. If you have already started with Amoxicillin and clavulinic acid, you can continue that for 5 days.

- I hope you have already taken you TT shot if you have not taken it in the last 6 months.
