Suggest Treatment For Small Blister Filled With Pus On Anal Region

Question: In last month i start to feel a small blister kind of thing on my anal region. Which is very small in the beginning and there was no pain for 3-4 days but after that it started to irritate me and there was a very mild pain when i sit. I have gone to a doctor he recommended me diltiazem 2% gel and said that you have anal fissure and it will heal in 2 weeks or so. But nothing happens in those two weeks. he also recommend me sitz bath but nothing happened the size of the blister doesn't reduce at all though diltiazem and sitz bath gave me relief for the time being. after 2 weeks it start to itching in that area and swelling also. then i go to a second doctor which recommend me a antibiotic cream which has ciprofloxacin, clotrimazole, neomycin, fluocinolone acetonide and some antibiotic tablets plus other drugs for 10 days that treatment gave me relief from itching and swelling in 10 days. but after that i can feel 4-5 blister on both side of the anal opening and i am having pain while sitting. i am not able to work properly. now that doctor gave me an ayurvedic medicin for piles. but its not working. i am still taking sitz bath and start applying Diltiazem which giving me some relief from spasm and burning while sitting. what to do please help me. thanks
Note-
i have constipation previous to that blistor episode and i also have joined a gym at that particular time.

The picture clearly shows pus pointing, about to brust. You are having an peri-anal abscess or as they are multiple and the area is quiet hairy, it can even be pilonidal sinus with abscess.

I would suggest that you show yourself to a good surgeon, today itself, for a physical examination as that is the only sure shot way of diagnosis. The treatment for both is incision and drainage under anaesthesia.

Till then you need to start on an antibiotic and painkillers of symptomatic relief.

initially the colour of the blisters was bluish black only now they turned yellow.
Plus the area where these blisters situated is not hairy at all.

The pus before reaching the skin has to make its way through the deeper layers, so initially the colour was bluish black when it was a little away from the surface. Now it has reached the surface so you can see its correct colour.

And the hairs in the region have shed off following the infection, so the area now is hairless.

i havn't seen any blood on my stool and all. so does it mean i don't have piles and fissure? I have never seen any cut at all as they say, people have in fissure and neither that amount of pain. The pain was only a mild one always and it goes worse when I sit.
Well, today I am going to see a surgeon but also tell me whether i see a general surgeon or the specialist one or any MD?

MD qualified doctors are physicians. They don't operate upon patients. The one who operate have MS degree like general surgeons.

You can consult a general surgeon. They are the specialist for these cases.

And if there is no blood in stools and there are no cuts in the area then chances of piles and fissure is very less.

Dear XXXXXXX Sir
I have gone to the General Surgery section of AIIMS Raipur. They told me that i am having fungal infection due to the cream i am applying have the steroid in it.
they prescribe me Sucral ano ointment and told me to take sitz bath daily 3 times a day.
I have applied the cream since last two days it giving me relief but as i have to apply with the help of probe its kind of causing irritation inside the anal canal.
i am also attaching the report of AIIMS with my question. Please tell me do they recommend the right prescription and why it is causing irritation inside the anal canal?

Hi XXXXXXX

First thing, the ointment is having a local anaesthetic as one of its component, and still you are having itching, could suggest that you might be having some reaction to one its constituent. If the itching is getting worse, i would recommend that you discontinue its use.

Second thing, they said that you are having fungal infection, but they didn't prescribed any antifungal cream. This is a bit strange.

Third, as they did complete examination and didn't found any perianal abscess or fissure, so that rules out these conditions. But from the picture you had sent, there is some infection surely present. That could be skin infection then like furuncle or boil.

So I would suggest that you take a course of oral antibiotic like a combination of ofloxacin and ornidazole like one available by the name of 'O2'. Its easily available over the counter. Take it twice daily for five days. This should take care of the infection. That much should suffice, no need for any ointment or sitz bath. You can also take an painkiller for antiinflammatory action.

The irritation, feeling is like something stuck inside the anal canal and not coming out its not the itching and Its not on the surface where the boils present. Please suggest me any cream otherwise I will go to AIIMS tomorrow again and ask them or should i apply the cream on the affected area only.
and yes i asked them about the boils they said they will go away themselves. i also asked 2-3 times that do these things needs surgery they said straight no.
Sir, what should i do? and also prescribe me something so that my bowl should be soft.

Hi,

Take any laxative to smoothen your bowel motions like syrup cremaffin or looz. 15-20ml in the night. Increase or decrease as per the response. Also eat 1-2 bananas daily and fiber rich diet like oats, whole grains, green vegetables.

But i can't understand where you want to apply the cream, as you want one. Because you are neither having any itching at the boils nor at the anal opening.

All you have is boils and hard stools. So you can take laxatives and oral antibiotics and antiinflammatory and high fiber diet.

I want to apply on the boils itself, which are just outside the anal opening(on the folds). Actually the cream I am applying comes with a probe kind of thing which i have to insert into the anal and then press the cream tube to apply, the probe have holes in it to release the drug inside. The doctor tells me to do so to apply inside the anal as well. but that causing problem as i described earlier.
i will start the medication you told me and i am having 1 laxative with me named Lactihep so will take that. now please tell me about the ointment?
