Brief Answer:
Combine with paracetamol or stronger meds, warm compresses

HI, thanks for using healthcare magic

Though the ribs are not broken, both them and the surrounding muscles may be inflamed.

If he it is not contraindicated (no medical reason that he cannot use), then he should consider an anti inflammatory pain killer such as cataflam, naproxen if the ibuprofen is not helping.
These should be used with a meal. Augment with paracetamol/tylenol since these are a different family of medications and can work together to help with pain control.

If none of these are effective then his doctor may consider a short course of opioids if he can use these medications.

Warm compresses instead of cold , since it is old injury, would also be helpful.
He can use a warm compress for 15 to 20 mins, 2 to 4 times a day and see if it helps.

I hope this helps, feel free to ask any other questions
