A problem that persists for 20 years is something chronic. If it hasn't gotten worse or better during all these years, it is less likely to go away by just a simple treatment. You need to find the root of the problem - Consider a general check up and usual blood tests to rule out anemia, a problem with the platelets or thyroid problems.
If all these result normal, then it can be a permanent damage of the skin in the past which now need to be kept under control - using the moisturising cream, being properly hydrated, using suncream when outside and using a hat, avoiding rubing the skin with rough towel, ect.
If the skin becomes inflammed, with blisters and itchy that means that the dry skin has flared up into an eczema and you can use a steroid cream on it - Hydrocortisone 1%, very thin layer, once per day and not longer then one week because the skin there is very delicate and can be damaged more.

It is a bit difficult to suppose what exactly may be causing the problem without examining it in person (I hope you'll understand).
I am supposing it is eczema caused by the hypersensitivity of your skin, so that is why I suggested the hydrocortisone. This may be the problem in 90% of the cases.
Also, I suggest checking the hemogram and TSH levels because platelets problems and thyroid problems can cause these symptoms.
Please check these first and I think it will give you/me an overall idea of the most common causes of the skin problem.
