Suggest Treatment For Spreading Rash On The Lower Leg

Question: Hi, Doctor, I've been having this skin problem on my outer lower leg above the ankle for the last two months. Initially it started as a scratch which i aggravated by scratching it some more & then putting aftershave lotion on to it. Theafter the scratching started forming a mark & then formed a rash. To address the problem , i started applying canasten on a regular basis which did not bring any relief. Thereafter from scratching the sensation turned into a burning sensation which began to pain. Thereafter, i stopped using canasten & switched on to suthol which did not give any relief.On 12th of May i visited a dermatologist in my home town as the psi had began to irritate & was hurting when ever rubbing against my jeans. By the way i generally wear jeans & travel a lot at times. The dermatologist said that this was an allergy & prescribed the following medicines - Lorapil 10 for oral consumption for 4 weeks & Clobial-S ointment for external application on affected area. The main appoint of origination as become better but the surrounding area has darkened & sometimes scratching & burning sensation is felt intermittently on the affected surrounding area. Attached snaps for your reference. Its been tow months now & i am not seeing a very positive progress on this i spite of the medication. Do i need to consult a better dermatologist or do i need to change my medication both orally & external application. Thanks.

Eczematous dermatitis; topical potent steroid+antibiotic cream

Detailed Answer:
Hi.

Thank you. I have reviewed the attachments.

It is infected eczematous dermatitis.

The ointment that you are already using contains clobetasol propionate 0.05% + salicylic acid 3% combination ointment.
If I was the treating doctor I would rather suggest you to use a topical potent steroid+antibiotic combination cream, twice daily for 2-3 weeks e.g tenovate Gn cream, contains clobetasol propionate 0.05%+neomycin sulphate 0.5%.

Topical steroids are prescription drugs and should be used under supervision of a dermatologist.
You should also follow it up with your dermatologist every 2-3 weeks.

In this Image I can see some oozing/crusting as well which suggest that there is some superimposed bacterial infection as well, due to scratching, therefore I suggest that you also be started on an Oral antibiotic e.g Amoxycillin thrice daily for a week.
Oral antibiotic would take care of the secondary bacterial infection.
Oral antibiotics are prescription medications.

Kindly visit a dermatologist in your vicinity for a review clinical examination and for necessary changes to treatment that need to be made.

An oral antihistamine e.g cetrizine 10 mg once daily will provide you symptomatic relief from itching.
