Brief Answer:
Visit ER - can be Shingles or some neurological affection.

Detailed Answer:
Hi.
Thanks for your query, albeit short and specific.
To recapitulate: Female/58 - very bad - burning stabbing pain - under upper back rib case - used Biofreeze, Salonspas Patch, Icy hot - no relief ...

If there is no history of any trauma, the most probable cause can be Shingles that is Herpes zoster.
There may not be development of classical rash so soon, takes time sometimes.
Please visit the ER asap so that a clinical evaluation can be done and proper management started sooner.
Please do not apply anything locally at the moment.
You may take an anti-inflammatory with painkiller properties like Ibuprofen if it suits you or any other painkiller that you have at home.

I hope this answer helps you.
