Suggest Treatment For Stinging Pinch In The Right Inguinal Region
Question: I am a 62 year old male. For the last few months, I have been experiencing a minor, internal pin-like, pinch-like pain, in the right inguinal region. Can't tell, if it's emanating from the external or internal ring area. I saw my PCP, about a month ago, but she could not find anything. As I've said, its minor, nuisance pain (say a scale of 2), over a very small area, like a faint pin-pricking sensation. Usually feel it, when I'm sitting. I was just curious , as to what the cause, could be.

Such a pain usually comes from one of the following reasons:
- Stretch of the nerve by the advancing hernia development where the hernia is not fully formed to be detectable by the classical impulse (bulge) on coughing or straining.
- Nerve pinching of the area you are having sensation with.
Proper examination can tell the diagnosis.
Put a hand on the painful area and see if you can feel any small bulge on forceful coughing or straining.

I did, as you suggested. I stood up, pressed a hand against the area, from which the pain emanates, and coughed forcefully. I did not feel any bulge, but I did experience, slight pain (again in a small area), when my hand, was pushed against the problem area. Again, it is not great pain - I was just concerned, since, though it is minor, it seems to persist. I am scheduled to see my doctor, in seven weeks, for a follow-up appointment.

This looks to be a local neuralgia, like a nerve pinching or local fibrous tissue formation in the muscles.
Clinical evaluation by a General Surgeon and high resolution of the abdominal wall can help.
Clinical examination should include the tests related to the lower thoraco-lumbar spine too, to rule out any problems.
