Suggest Treatment For Sudden Fever With Shivering In Child

Question: Hello doctor ,
This is Sunil .my 2.5 years old daughter this evening suddenly has temperature of 102 c and now she getting shivering and temperature is back to 101.3 c .with crocin her temperature was normal but now after 4 hrs she has this shivering an temperature as well.are these symptoms of viral .kindly advice

Dear XXXXXXX this is our 2nd interaction in this website. I remember your name very well and had counselled you regarding viral illness and I am glad that in your next question you have asked if it is viral or not.

I feel this may definitely be viral illness again as the season is changing and it is quite common for kids to get frequent viral illnesses in this season and age group.

Another issue is that it only few hours of fever. In viral illness it is very common for fever to be very high and with shivering especially in the 1st 48 hours of starting.

Shivering is quite common phenomenon with fever of more than 100F. As I have already suggested, you can give, Paracetamol can be given in the dose of 15mg/kg/dose every 4-6th hourly that too only if fever is more than 100F.

Regarding shivering - it is body's physiological response to mild raise in body temperature. It usually happens when you do cold sponging for fever. You need not treat shivering.

I have a few questions for you -
1. Apart from shivering what other symptoms does she have?
2. Is there any burning sensation or pain while passing urine?
3. Is there any pain in the tummy?
4. Is there a malaria endemic going on in the area you reside?
5. Did you do cold sponging for fever and after which this shivering started?
6. What is her current weight - so that I will suggest the dose of paracetamol.

Thank you doctor for ur quick response . She has no other symptoms of pain or malaria around the area and didn't give her cold sponge .

Hi XXXXXXX Good to hear that malaria is not endemic in your area. If you have not done cold sponging then the shivering could be due to high fever as I suggested earlier.
As you have not mentioned her weight, I suggest you use Paracetamol. Paracetamol can be given in the dose of 15mg/kg/dose every 4-6th hourly that too only if fever is more than 100F. I suggest not to use combination medicines for fever, especially with Paracetamol. NEVER USE MEFENAMIC ACID FOR FEVER EVEN IF SOMEBODY SUGGESTS YOU.
