
Suggest Treatment For Swelling And Pain In Calf And Foot

my wife 35 yrs old is having swelling in her calfs with severe pain in calf and foot, and is not even able to walk, which never happened earlier in winter. Though she has some history of rhumediod artheritis, which was been treated from time to time and never resurfaced from quite a long time.
We stay in XXXXXXX where winter is not that impactful, but is been felt around. could you advise some curative to overcome this pain please.

The calf swelling is usually not a good symptom to ignore for long. Especially if there is a history of rheumatoid arthritis.
I do want to ask a few questions to help reach the diagnosis:
1. Is the swelling on both the legs or just one?
2. Is it painful on touch?
3. Any knee swelling?
4. Is it present in the night time also when she sleeps?
5. Has her cpk and thyroid levels been checked?
6. Any symptoms in the arms or forearms?
Do let me know these queries. With calf swelling it is very important to see if the swelling is due to muscle inflammation or a clot in the veins there. Sometimes calf swelling can happen in rheumatoid arthritis due to bakers cyst rupture. So if it is really swelling we need to look into these causes. 1 yes the swelling is for both the legs
2 the calf muscles are painfull on touch.. as massasing with oil or so causes her more pain
3 yes there is a bit swelling in the kneee
4 yes the pain is persistent during night time also
5 thyroid is checked last year and was found ok...pls advise on cpk
6 yes thwre is swelling in arms and fore arms and some time pain in muscles . when some work at home is done
on the first day she had mild fever which subsided with paracetamol. her palms also pain some time. kindly advise further

Well, I think you should consult a Rheumatologist. She looks to be having polyarthritis going on, with a probable muscle involvement. The knee swelling along with calf needs to be looked into. Especially the pain at the night time which does not subside with rest does look alarming and something which needs to be worked up for.
CPK is a muscle enzyme to suggest if any inflammation is going on in the muscles. It normally gets aggravated if there is damage to the muscles. I would want to re run her tests once more to see where exactly the inflammation lies.

Is it Fibromyalgia.

Well I have gone through your prescriptions and it looks like none of the doctors think it is rheumatoid arthritis. The medications they have prescribed are for Fibromyalgia. It is a condition where there is pain all over the body with a feeling of swelling in multiple parts as well. There is no objective signs visible outside. There is lower threshold of signals coming from the brain, so a patient tends to experience heightened response to any pain stimulus.
The only abnormality I found was ANA +ve as mentioned by one of the doctors. What I would recommend you, is to get ENA Profile done as well. It will further check all the subset of antibodies linked to ANA. Just incase, we shouldn't miss anything. I am not too sure if she experiences symptoms of dry eyes and dry mouth as well.
As for Fibryomyalgia, it is the diagnosis of exclusion. So you should rule out all other causes before making it as a diagnosis as the management for it will differ in many aspects.
